Overview

The Skye, Lochalsh and Wester Ross Social Work Team has a vacancy for a Health and Social Care Co-ordinator. This is a permanent post that could potentially be based in any of the social work offices in the District, namely Portree, Broadford or Ullapool. Location would depend on the preference of the successful candidate.

There are currently 2 HSCC’s in post, one full time post in Broadford and one part time post in Ullapool, and the intention is to further strengthen this important role by being able to provide a full week service including ensuring that all annual leave can be covered.

Responsibilities
  • HSCCs have a very broad role including coordinating referrals to integrated adult services such as social work, Care@Home, physiotherapy and OT, dealing with and sign posting enquiries from the public, supporting the daily discharge meetings and conducting basic assessments. A key part of this role is enabling early intervention of services to facilitate preventative care.
  • As part of the social work team we work closely with other disciplines to provide a multi-disciplinary and integrated service for adults. We deliver a high standard of flexible and responsive social work interventions to help ensure the best quality of life for the people we support.
  • This post plays an important role in helping ensure the wellbeing of the communities and people we support to lead fulfilling lives.
Requirements / Skills
  • While much of the job will be office based there may be a requirement to undertake home visits for basic assessments. Good IT skills are required and even more importantly the ability to work as part of a close knit team of Health and Social Care Co-ordinators that is mutually supportive. You will also need to be able to work closely with the social work Team and other professionals.
  • Good people skills are essential and some experience of social care would be an advantage.
  • You will be provided with a full Induction and training as part of your continuing career development. This is an excellent opportunity to experience working in a busy social work environment and would be ideal for someone thinking of further career progression in that area.
  • Due to the rural nature of this post a driving licence is advantageous but not essential.
