We are seeking a highly skilled and motivated Emergency Care Practitioner (ECP) or Advanced Clinical Practitioner (ACP)to join our friendly and committed Primary Care team at Lister Medical Centre.

The successful candidate will be responsible for conducting advanced clinical assessments, diagnosing, treating, and managing patients with undifferentiated and undiagnosed conditions.
A strong background in Primary Care is essential, along with excellent interpersonal skills and the ability to work effectively under pressure. Familiarity with SystmOne is desirable.

We are a well-established, friendly teaching practice, andour dedicated team includes 8 GPs, 4 Emergency Care Practitioners, 3 PracticeNurses, 2 Nursing Associates, 2 Physician Associate, 4 HCAs and 17admin/support staff. We have been providing a full range of GP services forover 30 years and have a patient list size of over 21,000 patients.

Job Summary
To practice as an Emergency Care Practitioner(ECP)/ACPandsupport the on-call team in delivering a same-dayemergency care service for patients. The role involves the assessment,diagnosis, treatment, or referral of patients presenting with undifferentiatedand undiagnosed conditions. The post holder will operate within theirprofessional scope and competencies.
KeyResponsibilities
Clinical
To provide assessment, diagnosis and treatment at first point ofcontact by attending to patients according to patients needs either inthe Practice or patients homes.
To support and develop the Practices same-day emergency careservice.
To evaluate clinical information from examination and historytaking and initiate appropriate treatment and/or referral.
To signpost appropriately to other services within the defined healthcarepathways.
To refer patients to appropriate care settings, within locallyagreed healthcare pathways.
To maintain accurate, timely and systematic documentation ofpatient interactions and clinical care provided, including adverseevents.
To offer patients health promotion advice, support for healthylifestyles, and disease prevention strategies.
To provide clinical advice and support to other team members where appropriate.
To provide effective chronic disease management for patients, including conducting home visits where required.
Communication
To liaise and interact with multi-professional teams acrossorganisational boundaries, including primary care, social services andsecondary care providers according to patient needs and appropriate carepathways.
To maintain communication with all healthcare professionals and outsideagencies to ensure seamless, patient-centred service provision.
To ensure effective communication so that thepatient receives the appropriate level of care and wherever possible avoidunnecessary hospital admission.
To take an active role in maintaining expertcommunication with all patients, carers and relatives, tailored toindividual needs.
To deal with complaints in a calm andcourteous manner, ensuring that, wherever possible, complaints are dealtwith efficiently and satisfactorily on a local level.
To communicate effectively in demanding andchallenging situations, where there may also be hostility.
Professional
To work collaboratively with other members of the health care team such as the District Nursing Team, Mental Health Services, staff in Care Homes, Ambulance Service and Secondary Care.
To adhere to all practice policies to supportthe health and wellbeing of patients and staff.
To undertake clinical audits as requested in line with theorganisations clinical audit plan, and to participate as required in otherresearch.
To contribute to the analysis of information to inform futuredevelopment of the service.
To participate and contribute to relevant clinical meetings.
To identify risks to the provision of the service and highlight these tothe Clinical Lead.
To ensure compliance with Data Protection legislation concerning patientrecords.
To maintain current professional registrationand continuing professional development.
To act at all times in such a manner as to promote confidence and public trust and to uphold the reputation of Lister Medical Centre and their own profession.
Organisational
To operate as an effective and reliable memberof the Practice team.
To maintain accurate and systematicdocumentation of patient interactions and clinical care provided,including any adverse events, child protection issues or other notifiablematters.
To provide information to inform theevaluation and efficiency of the role and for performance managementpurposes as requested.
To engage in self-appraisal and utilisereflective practice to enhance and develop personal and organisationalpractice.
Managerial
To ensure effective time management andprioritisation of workload.
To ensure the security, safe storage andmaintenance of any allocated equipment and goods.
To ensure practice is within good clinicalgovernance and based on sound up-to-date clinical evidence. Whereappropriate, to identify and assess any risks associated with patient careor to oneself and communicate these risks to the appropriate clinical manager.
