Vacancy

Deputy District Nurse Team Leader vacancy within the Adult Community Services Department to provide nursing care to allocated patients within their homes.

Responsibilities

• The post holder will be responsible for the planning, implementation and evaluation of nursing care needs and the delivery of high-quality and up-to-date care to allocated patients in their own homes and/or a variety of settings.
• The post holder will work within a team for a defined number of patients, and will work under the supervision of the Clinical Operations Manager.
• You will be responsible and accountable for the management of a caseload in the absence of the Clinical Operations Manager/District Nurse.
• You will also work closely with the aligned GPs to your team and will be both the advocate and the key channel of communication for their care.
