Job summary

A leading elderly care provider is looking for a Dementia Care Manager in Edenbridge. You will oversee the management of the Dementia Community, develop care plans, and work closely with families and the Activities Team to enhance residents' quality of life. The ideal candidate will have at least three years of experience and knowledge of the UK health and social care market. This position offers a salary between £33,000 and £35,000 annually and is a full-time, permanent role.

Job description
Overview

The Dementia Care Manager will be responsible for the overall management of the Dementia Community at Avery Healthcare, implementing and embedding the company's dementia care strategy and managing resources within a given budget. They will build caring relationships with residents and families, work closely with the Activities Team, and promote a culture that enables residents to maximize their capabilities and enhance their quality of life.

Responsibilities
  • Ensure the plan care day is accurate and reflected in the care needs summary; ensure information is integrated into a person-centred care plan, with residents/families identifying needs and preferences.
  • Build caring relationships with residents and families, liaise with them sensitively, act as an advocate for residents\' best interests, and appropriately manage conflict.
  • Work with the Activities Team to develop meaningful activities for residents with dementia and set expectations for the care team\'s participation.
  • Promote a culture that enables residents to maximise their capabilities, optimise independence, and enhance quality of life.
