A health organisation in Wales is seeking an experienced Community Rehabilitation Practitioner in Denbigh. You will provide rehabilitation services within a multi-disciplinary team focused on supporting service users' return to their home area. The role requires registration as a Band 6 Practitioner, preferably with experience in Mental Health or Social Work. Knowledge of Welsh is desirable. Applications are welcomed in both English and Welsh.

Job Responsibility:
An exciting opportunity for an experienced Registered Band 6 Practitioner to work within a dynamic multi-disciplinary team, providing Community Rehabilitation Services to the Mental Health and Learning Disability Division. The team is based in Denbigh and provides rehabilitation focused services for service users within NHS rehabilitation inpatient units, Out of Area placements, and the community.

The aim of the team is to provide a focus on the repatriation of service users back to their home area and to provide intensive community rehabilitation. This post is open to experienced registered practitioners, such as Mental Health Nurses and Social Workers, who meet the requirements of the post. The ability to speak Welsh is desirable.
