As a Clinical Pharmacist within our Primary Care Network, you will play a central role in supporting patients to get the very best from their medicines and in helping our practices deliver safe, effective, and high-quality care. You will work as part of a multidisciplinary team, providing expert advice on medicines use, carrying out structured medication reviews, and supporting patients, particularly those with long-term conditions, polypharmacy, or complex health needs.
You will contribute directly to patient care by improving understanding of medicines, optimising treatment, and promoting self-care and prevention. You will also take responsibility for areas such as medicines reconciliation, repeat prescribing, and prescribing safety, as well as supporting national and local initiatives including antimicrobial stewardship and long-term condition management.

Hoddesdon and Broxbourne (H&B) PCN has an exciting opportunity to further expand their Clinical Pharmacist workforce and are looking to take on another PCN Pharmacist to work across the GP practices within H&B PCN. This is to be a 1 year Maternity cover initially, with the possibility to become permanent.
H&B PCN is a forward-thinking network comprising four practices within close proximity of each other with a total patient population in excess of 42,000. We would like to build on our existing team of four Pharmacists as we continue to recognise the value this role brings to our practices and patients. Our aim is to provide exemplary patient care; find innovative solutions in general practice to deliver the best care we can for our patients.
The PCN has incorporated H&B Health Ltd which will act as the provider company supporting the PCN business. This will better facilitate the standardisation and coordination of the PCN workforce as well as offer opportunities to further develop services which support and enhance our Primary Care model of care.
SCOPE OF THE ROLE
We are seeking an enthusiastic and forward-thinking pharmacist to join our likeminded PCN. We want to find the best pharmacy talent to join our team. You will be part of a group of PCN Pharmacists working under the supervision of the Clinical Director delivering care in line with the PCN DES specification and agreed specific service offer to our member practices.
It is a requirement of the role to be registered with GPhC and have a minimum of 2 years post-registration experience. Successful candidates will be supported in terms of clinical learning through access of the PCN Clinical Director, and fellow Pharmacists within the PCN.

Salary expectations are based on experience, and an NHS Pension Scheme is available.
A full driving license is essential as travel between sites is necessary.
Job Summary
Key responsibilities and accountability
Structured Medication Reviews
Conduct face-to-face, telephone, domiciliary and virtual (where appropriate) structured medication reviews to proactively manage patients with Long Term Conditions
Proactively identify and manage patients with complex polypharmacy
Review the ongoing need for each medication, ensuring monitoring needs are performed and an opportunity to support patients achieve better health outcomes
Reduce inappropriate polypharmacy and wasteful prescribing
Medicines Information
Provides timely and accessible answers to relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams (e.g., community pharmacy) and patients with queries about medicines.
Suggest and recommend solutions using clinical knowledge and trusted pharmaceutical sources.
Medicine Reconciliation
Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post-discharge.
Review medications for newly registered patients
Proactively transfer workload relating to medicines optimisation from other clinicians to the pharmacy team to improve patients safety and therefore freeing up other clinical staff for clinical care
Medicines Management
Produce and implement a practice repeat prescribing policy helping to improve safety and efficiency
Develop and implement ways to improve monitoring of high-risk medications (HRDs) such as anticoagulants, anticonvulsants, and DMARDs
Monitor practice prescribing against local guidelines and formulary recommendations
Develop prescribing guidelines for use within the Network
Help increase uptake of Electronic Repeat Dispensing
Audits
Medicine Safety
Implement changes to medication that result from CAS/MHRA alerts, product withdrawal, and other local/national guidance
Develop and implement systems to identify cohorts of patients most likely to be at risk of unplanned admissions and readmissions from high-risk drugs or polypharmacy
Collaborating Working Relationships
Work collaboratively with the PCN team
Develop relationships and work closely with community, hospital services as well as community pharmacy colleagues and the wider health system
Liaise with other stakeholders for the collective benefit of patients
Attend relevant network and practice meetings
Education and Training
To analyse latest prescribing information and trends and subsequently present this in an appropriate format to clinicians within practices to promote good prescribing practice
Provide specialist education to pharmacy colleagues, nursing staff and GPs on aspects of medicines optimisation
Demonstrate self-development through continuous professional development
Implement strategies to support antimicrobial stewardship
Practice Targets
Support practices to deliver on QIPP agenda, QOF, ECF and locally commissioned enhanced services
To be aware of the prescribing budget and identify and action areas for cost improvement
Work with PCN team to ensure the practice is compliant with CQC standards where medicines are involved.

Risk Management
In accordance with the Risk Management Strategy, employees will participate, whenever required, with the risk management process. They will support line managers by attending mandatory and statutory training, completing incident/accident forms for every adverse event or near miss that occurs, report all defects and complaints, and communicate any dangerous situation to individuals potentially at risk.
