CHC ADVISOR ROLES AVAILABLE WITHIN OUR LANCASHIRE AND SOUTH CUMBRIA FOOTPRINT. The post is family‑friendly and open to all allied health professionals and nurses.

The post holder will work within the Continuing Healthcare Team concerning the commissioning and reviewing arrangements associated with our Child and Adult continuing healthcare funded placements. The post will lead on Best Interest decisions and Court of Protection cases as necessary, liaising with relevant agencies as required for CHC funded patients.

The role combines agile working with office‑based tasks and visits to patients in their homes or placements.

Main duties of the job
  • Support the assessment and review process for NHS Continuing Healthcare (CHC) and NHS Funded Nursing Care (FNC); conduct comprehensive person‑centred assessments to determine eligibility.
  • Work autonomously and collaboratively with individuals, representatives and a multi‑agency environment.
  • Maintain a caseload and conduct reviews of CHC funded patients as required.
  • Engage in professional development and participate in the ICB’s learning opportunities.
Key Responsibilities
  • Monitor progress on care, inform future commissioning arrangements and assist in identifying future resource implications.
  • Ensure informed information is available for commissioning, investigate sources of referrals, and assess compliance with ICB commissioning policies.
  • Support patients to achieve effective, high‑quality care in an appropriate location and optimise use of ICB resources.
  • Benchmark NHS and non‑NHS provider services, analyse National Care Standards Reports and other Clinical Governance data.
  • Represent the ICB at provider visits, review meetings, Court of Protection hearings, and produce relevant reports.
  • Maintain and update Case Management on CMS following each patient contact and keep abreast of Department of Health guidance and procedures.
  • Participate in improvement of services as directed by the Quality Monitoring Team and line manager.
  • Implement safeguarding and maintain contemporaneous clinical records in line with Trust policy and NMC standards.
  • Comply with NMC/AHP revalidation requirements and maintain professional portfolio.
  • Maintain constructive relationships with internal and external stakeholders, working groups, and initiatives.
  • Develop data collection systems to supply accurate, timely information to ICB senior management.
  • Communicate risks, issues and dependencies, and provide briefings and reports to senior managers and staff.

Person Specification
Knowledge
  • Previous experience in a clinical role with thorough knowledge of clinical settings and pathways.

  • Clear communicator with excellent writing, report‑writing and presentation skills; capable of delivering clear ideas concisely and accurately for diverse audiences.
