About the role: This is an exciting opportunity for an enthusiastic and dynamic Care Coordinator to join the LEO Community Team. The post is open to nurses, OTs and social workers.
The post-holder will conduct assessments of newly referred adults to the Early Intervention Service who are experiencing First Episode Psychosis (FEP). They will undertake the responsibilities of a care coordinator within the Care Programme Approach for a defined caseload of service users and their carers. They will provide evidence-based psychosocial interventions and will focus on engagement, recovery and social inclusion as well as addressing physical health care needs.
The successful candidates will be experienced registered professionals who are able to demonstrate a specific interest in working with people with developing psychosis and who have either completed preceptorship or evidence of band 6 skills.

Main Duties of the Job
- Assessment: Responsible for the assessment of newly referred adults with First Episode Psychosis and the subsequent presentation of findings to the wider team. Work with diagnostic uncertainty, providing ongoing comprehensive assessments and outcome monitoring for all service users within caseload. Undertake risk assessments regularly, involving all other health professionals/agencies. Ensure risk assessments are regularly updated, meaningful and reflected in care plans.
- Engagement: Assertively outreach difficult-to-engage clients. Meet service users regularly, preferably at a venue of their choice, such as home, CMHT centre, educational or work or recreational settings to assist with engagement and encourage participation in treatment delivery and independence in daily living.
- Support: Provide ongoing care coordination of a designated caseload of service users who will be experiencing their first episode of psychosis. Provide support and advice in relation to service users regarding the issues concerning them. Work collaboratively with service users focusing on a recovery approach and provide information to help clients make informed choices.
- Interventions: Provide interventions relevant to a first episode client group. Ensure all interventions are age appropriate and sensitive to the needs of black and minority ethnic service users. Provide low intensity CBT and Mindfulness interventions under supervision and psycho‑education to service users. Maintain a client‑centred approach with a focus on resocialisation and social inclusion.
- Families and Carers: Involve families and carers in treatment plans where appropriate. Provide direct family interventions, ensure services are delivered within guidance of the Children and Carers Acts, assess the needs of carers and offer appropriate support and psycho‑education.
- MDT Working: Work in cooperation with members of the multi‑disciplinary team and other relevant health professionals to assess, develop, implement and evaluate high quality client‑centred care packages. Form effective relationships with inpatient services to facilitate early effective discharge and participate in all clinical meetings.
- Communication: Maintain accurate clinical documentation on designated caseload, keeping case notes up to date. Ensure effective verbal and written communication with all involved in the delivery of care, liaising with other disciplines and organisations to manage and communicate all issues affecting service user care to a high standard.
- Personal Development: Keep up to date with current and emerging good practice in community mental health services, including Early Intervention services. Abide by policies of SLAM NHS foundation Trust and Local Authority. Meet regularly with Team Manager/Supervisor and undertake mandatory training and CPD as agreed with Team Leader. Provide supervision to junior staff.
- Miscellaneous: Participate in team duty system, provide cover for colleagues in their absence, participate in research as requested, undertake audit/clinical governance duties and other tasks as requested by Clinical Service Lead and LEO Team Leader. Willing to work from any location in the borough.

Job Details
Band 6; Fixed term; 12 months; Full‑time; Salary £46,419 to £55,046 a year inclusive of HCAS.

Qualifications
- Relevant professional qualification (e.g. RMN, CQSW or equivalent).
