A GP practice in Addlestone is looking for an enthusiastic Care Co-Ordinator to join their team. This role focuses on improving patient care through managing prescription requests and supporting the clinical team. Main Responsibilities:
The Crouch Oak Family Practice in Addlestone is a dynamic, friendly, and supportive GP surgery caring for 17,500 patients. Our multi-disciplinary team of around 70 staff works collaboratively to deliver high-quality, compassionate care to our community.

Key Responsibilities:
To ensure all medicine requests and queries are dealt with in a polite and confidential manner
To ensure medicine requests from patients, hospitals, pharmacies and care home are dealt with and added to the signing list- medication query list in an efficient and timely manner following the Repeat Prescription Request Process in the Medicines Management Policy
To support the management of requests sent through the Practice Website/NHS App
To ensure all patients have a nominated Pharmacy where possible
To ensure Prescriber is correct and updated / re-authorised as appropriate
To highlight any queries or requests for medicines not on patients current list to Pharmacy Technician or Clinical Pharmacist as appropriate
To highlight any urgent requests for medicines to Pharmacy Technician or Clinical Pharmacist as appropriate
To highlight any patients requesting medicines too early, irregularly or inappropriately to Pharmacy Technician or Clinical Pharmacist as appropriate
To ensure need for blood tests, blood pressure and / or weight is reviewed and relevant appointment is made or highlighted to Pharmacy Technician or Clinical Pharmacist as appropriate
To highlight need for medication, respiratory, diabetic, contraceptive or similar review to appropriate Clinician, Pharmacy Technician or Clinical Pharmacist as appropriate
To ensure repeat medicines are synchronised whenever possible to minimise number of requests received from patient
To highlight any out of date medicines on repeat list to Pharmacy Technician or Clinical Pharmacist as appropriate
To ensure good communication with all our local pharmacies particularly regarding dosette box patients, supply issues and general queries
To provide administrative support for the GPs, Clinicians, Clinical Pharmacist, Pharmacy Technician, PCN Pharmacists and CCG Pharmacists where possible
To ensure the correct protocols are followed for reporting prescribing errors and ensuring an NRLS is completed
Any other duties as appropriate.
