Job description

We have a 30 hours per week Fixed term maternity cover position available within the Trowbridge Community Team, but we would consider part time for the right person, as we are looking for inspirational Registered Nurses to deliver a high standard of quality care to patients within the community setting. Wiltshire Health and Care is proactive in developing community specific training and can offer a supportive and friendly environment in which to learn new skills.

Community Nurses deliver care in patients' homes and residential homes as part of a Multi-Disciplinary Team, linking closely with the GP Practices and other community services in the local areas. We work with the aim of preventing unnecessary admissions to hospital and in assisting people to manage their conditions in their own homes.

The Trowbridge Community Team are a multidisciplinary team including Nurses, Therapists, Clinical Leads, the Home First Team, Care Coordinators and other staff. The team is based in Trowbridge Community Hospital and operates a shift pattern from 07.00 - 22.00. Nursing staff will be expected to work weekends and will be expected to cover early and late shifts. Evidence of up to date clinical professional development is essential and experience in front line NHS community patient care is preferable.

Applicants need to have excellent clinical skills, well developed communication skills and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly and efficiently.

You will put to use your strong communication and organisation skills, along with a flexible approach, to ensure you deliver positive experiences to patients at every stage of their healthcare journey.

  • Registered Nurse (RN) with current live PIN on NMC register
  • Teaching and Assessing NMC approved qualification

  • Experience of managing a range of long-term conditions
  • Experience of working collaboratively with Social Services
  • Evidence of innovative clinical practice development

As this role requires travel, a driving license and access to a car is required for this role.
