A local healthcare provider in Wisbech is seeking a motivated and skilled ANP/ACP to join their home visiting team. The role involves conducting assessments and providing care to patients in their homes, ensuring timely medical intervention to avoid referrals to GPs. Candidates must be registered with NMC/HCPC, a non-medical prescriber, and possess a relevant master's degree. Working as part of a busy team consisting of 3 other ANP's, you will be providing an efficient home visiting service. Working independently and autonomously through mainly face-to-face consultations in the patient's own home or care home. You will be accountable for delivering safe, clinical decision making and expert care to a high standard. Acting within your professional boundaries and scope of practice.
Your duties will include all (or part) of the patient journey, including history taking; clinical assessment, diagnosis, treatment/prescribing and review. The ANP/ACP role will encompass the four pillars of advanced clinical practice. The role is to provide robust on-the-day appointments and effective triage for our housebound complex patient list.
We are a friendly GP Practice looking for a motivated, highly skilled and enthusiastic ANP/ACP to join our home visiting team.
The ANP/ACP autonomous role provides an opportunity for patients to avoid the need to be referred to a GP, thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
The main duties of the role are:
Conducting home visits for patients who are unable to attend the practice (e.g. frailty, palliative care, acute illness, complex, chronic long-term conditions)
Assess undifferentiated patients with a range of acute, non-acute and chronic medical conditions.
Take history, make appropriate physical examinations, formulate differential diagnoses and carry through management treatment plans when working in a home visit environment.
Through telephone triage and clinical assessment, obtain a relevant and sequential history of the presenting complaint to determine a working diagnosis.
Ordering and interpreting blood tests, scans, x-rays, including other relevant investigations and referrals
Recognise the need and initiate emergency treatment to save a life or safeguard the patient's interests, and refer to emergency services when appropriate. This includes escalation/referral to hospital or other community services.
To be professionally and legally responsible and accountable for all aspects of clinical decision-making within your clinical competence.
Working alongside other ANP/ACP/GP's, community teams to provide holistic patient care.
Escalating complex cases to senior clinicians
Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual).
