Advanced Clinical Practitioner - Langley Medical Practice
Langley Medical Practice are a friendly, supportive and thriving practice looking for an Advanced Clinical Practitioner. This role is offered on a part-time or full-time basis and will work in collaboration with an excellent team of GPs, Practice Nurses and Health Care Assistants.
The practice is part of the S&C Primary Care Network (PCN) with two other practices who share space in the Health Centre. This provides excellent access to allied health care practitioners including FCPs, Pharmacists, GP assistants, Social Prescribers, and Mental Health Support Workers. The PCN also contributes staffing for extended hours services, providing an opportunity to potentially increase working hours.
The practice serves c9000 patients from Surbiton Health Centre, a modern purpose built premises, and is five minutes from Surbiton train station.
Langley are looking for an autonomous practitioner who has the knowledge, skills and clinical expertise to assess, treat, diagnose, and independently prescribe medicines. The role will include running clinics, triaging and managing minor illnesses, along with providing continuity for patients with complex health needs.

Main duties of the job
Clinical practice
- Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
- Assess diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice
- Work with patients in order to support compliance with and adherence to prescribed treatments Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Main duties of the role:
Clinical practice

- Support and manage health needs of women presenting for family planning, sexual health consultation as appropriate to your competence, and perform cytology if required.
- Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health.
- Implement and participate in vaccination and immunisation programmes for both adults and children if required
- Advise, support and administer vaccinations for patients travelling abroad, where appropriate
- Promote and deliver evidence-based care for patients presenting with aural conditions
- Meet the needs of patients presenting for opportunistic wound care if required
- Undertake minor surgery as appropriate to competence
Clinical Governance/Administration
- Understanding of CQC 5 domains, work with the GP partners and practice manager on gaps analysis, action planning, clinical audits, drugs monitoring, health campaigns, high risk patients with long term conditions diabetics/asthmatics, clinical safety, policy updates as directed.
