Reporting to the Director of Care, the Licensed Practical Nurse (LPN) is responsible for providing planned, patient-oriented, goal-directed nursing care in accordance with provincial standards of care.

1. Work as a member of the inter-professional team, applying a person-centered approach to care.
2. Complete and ensure resident's clinical care assessments, care/service plans, and documentation are accurate, up-to-date, and communicate changes as required.
3. Implement and evaluate the effectiveness of care/service plans and make necessary adjustments.
4. Assist with scheduling resident care conferences and participate when applicable.
5. Work closely with the Director of Care to facilitate resident move-ins, move-outs, and transfers.
6. Provide direct resident care, administer medication/treatments, and document care in compliance with standards, policies, and procedures.
7. Process orders, safekeep drugs and narcotics, and administer in accordance with regulatory requirements and policies.
8. Facilitate and provide resident and family education.
9. Plan, coordinate, manage, and evaluate the resident’s care plan in collaboration with residents, substitute decision-makers, physicians, and the interdisciplinary team.
10. Coordinate nursing team’s resident care assignments for efficient and timely service delivery.
11. Respond to and manage significant changes in residents' conditions or medical emergencies; provide emergency first aid.
12. Teach, delegate to, and supervise unregulated care providers.
13. Coordinate emergency response in the absence of the Director of Care or Director of Health & Wellness/Executive Director.
14. Address resident/family concerns, ensuring appropriate actions are taken and concerns communicated to the appropriate leadership.
15. Assist in the coordination, implementation, and monitoring of Infection Prevention and Control, Health and Safety, and Clinical Programs.
16. Maintain a position of trust with vulnerable adults (residents).
17. Uphold and promote the organization’s values, ethics, and integrity as outlined in the Code of Conduct.
18. Perform other duties as assigned.

• Current registration as a Licensed Practical Nurse in the applicable Province/State.
• Experience working in senior care environments and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, or community/long-term care are assets.
• Strong communication, critical thinking, care coordination, time management, and customer service skills.
• Demonstrated sound clinical knowledge.
• Current Basic Cardiac Life Support and First Aid certification.
• Proficiency with Microsoft Office applications.
• Provide a current (within six months) Vulnerable Sector Check, including Criminal Background Check, or willingness to obtain one.

Wage Leveling Salary: $31.89 - $40.15 per hour
