About the HART Hub
The London HART Hub is a low barrier recovery-oriented space supporting individuals experiencing homelessness with mental health and addiction needs. It offers access to high-quality, trauma-informed, culturally appropriate care and connects clients with a continuum of care, including transitional supportive housing and various mental health and addiction services.
Role of the Registered Nurse
The RN will utilize a holistic approach for client services, including assessment, treatment planning, and care implementation based on the bio-psychosocial model, in line with policies and practices. Responsibilities include supporting withdrawal management, addiction support, team-based care, safety strategies, and post-discharge support with linkages to other programs.

What We're Offering
- Starting salary: $32.13/hr (Level 1 of RN/OT Grid); progression to $41.41/hr up to Level 8 based on seniority
- Work schedule: 70 hours over two weeks, Monday to Friday, Days or Afternoons
- Benefits: 9% in lieu of benefits
- Pension: Immediate eligibility for HOOPP enrollment
How You Will Make a Difference
- Screening, assessment, and nursing supports for HART Hub clients
- Collaboration with addiction and mental health providers for crisis and withdrawal support
- Supporting intake and health assessments, leading addiction treatment plans
- Maintaining Medication Administration Records and managing medications
- Promoting HART Hub services and conducting assessments and referrals
- Assessing medication effectiveness and managing depot injections
- Supporting health measures and health promotion groups
- Identifying high-risk clients and crisis intervention
- Providing emergency care with dignity and confidentiality
- Collaborating with clients and support teams to develop care plans
- Acting as a resource and demonstrating sensitivity to diversity
- Adhering to standards, completing documentation, and ensuring privacy
- Supporting program development and additional duties as required
Application Requirements
- Police Vulnerable Sector Check (recent within six months)
- Baccalaureate Degree or Diploma in Nursing and registration with the College of Nurses of Ontario
- Valid Driver’s License, insurance, and access to a vehicle
- First Aid/CPR certification
- Demonstrated regular attendance
Preferred Qualifications
- ASSIST and Non-Violent Crisis Intervention Training
- Experience with electronic medical records (PS Suites preferred)
- Understanding of social determinants of health, addiction neurobiology, and community resources
- Knowledge of chemical dependency, bio-psychosocial care, and co-occurring disorders
- Bilingual in both official languages (preferred)
