Job description
UNIT PROFILE

Forest 1, Seniors Mental Health Unit is an inpatient program that employs a multidisciplinary team of psychiatrists, psychologists, nurses, social worker, occupational therapist, rehabilitation therapists and recreation therapists. This program assesses and treats older adults with serious mental illness, mood disorders, and late onset psychosis with age related issues, which complicates recovery. The Program optimizes a variety of proven treatment strategies (pharmacotherapy, group therapies, psychoeducation) with the aim of controlling the symptoms, as well as supporting the recovery and rehabilitation of the older adult.

Harbour North 1, Seniors Mental Health Behavioural Unit is an inpatient program that that employs a multidisciplinary team of Psychiatrists, Nurses, Social Workers, Occupational Therapists and Recreation Therapists. This program assesses, treats and manages individuals with a diagnosis of moderate to severe dementia who also have responsive behaviours associated with this diagnosis.

The Registered Nurse will be assigned to work as needed on the Seniors Mental Health Inpatient Programs of Forest 1 and Harbour North 1 Seniors Mental Health Inpatient Programs as per ONA sister agreement.

POSITION SUMMARY

The Registered Nurse – Mental Health and Addiction Programs (RN- MHAP) provides care for patients in accordance with the mission, vision and values of the SJHH and the Mental Health and Addiction Program (MHAP), the standards of practice of the College of Nurses of Ontario and the Canadian Federation of Mental Health Nursing (CFMHN) Standards. Mental Health Nursing is a specialized area of nursing practice, education and research. Nurses in the MHAP are committed to the alleviation of stigma and discrimination related to mental health and addiction. The RN-MHAP is committed to the reduction of the use of seclusion and restraint in all of our services. The principles of Person-Centred Nursing, recovery and trauma informed care is foundational to this specialized nursing practice. As a member of the inter‑professional team, the RN - MHAP is responsible for the provision of quality, holistic, specialized nursing care for patients with complex mental illnesses in Acute and Specialized settings. The RN – MHAP provides safe, evidence‑based therapeutic interventions across the care continuum. The RN-MHAP models professional leadership and provides decision‑making support by collaborating and delegating within the inter‑professional team. The RN-MHAP completes assessments, plans and organizes treatment/therapeutic interventions, documents and evaluates interventions for patients with complex mental illnesses in Acute and Specialized settings. QUALIFICATIONS
  • Current registration with the College of Nurses of Ontario required
  • C.N.A. certification in Psychiatric Mental Health Nursing, CPMHN(C), is preferred
  • Current Cardiopulmonary Resuscitation is required
  • CPI – Crisis Prevention Training (or successful completion as part of clinical orientation is required)
  • Demonstrated knowledge of P.I.E.C.E.S. framework (e.g. Long Term Care Homes, acute care, community-based services etc.)
  • A certification in Geriatric and/or Mental Health nursing preferred
  • 1 year recent experience developing and facilitating therapeutic groups
  • Training in evidence‑based treatments for older adults with serious mental illness, mood disorders, and late onset psychosis with age related issues, which complicates recovery, preferred (ie. CBT, DBT, Mindfulness, Behavioural Activation, Emotional Regulation, etc.)
  • Two years recent clinical experience working in mental health nursing.
  • One year of recent clinical experience with demonstrated knowledge, skill and ability in the care of older adults with serious mental illness, mood disorder and late onset psychosis with attendant age‑related issues which are complicating recovery is preferred.
  • Expertise working with complex family dynamics and delivering family centred care is essential.
  • Demonstrated knowledge and experience in the current treatment modalities of patients with severe and persistent mental illness, particularly older adults diagnosed with a mood and/or late onset psychotic disorder preferred
  • Demonstrated ability to develop effective therapeutic relationships and balancing need for adherence to therapeutic boundaries
  • Demonstrated ability to assess patients on an ongoing basis including risk for suicide, mental status, physical condition.
  • Demonstrated knowledge and application of the various clinical scales used for older adults with a mood disorder (GDS, Cornell), preferred
  • Demonstrated experience developing and facilitating groups preferred
RESPONSIBILITIES
  • Applies principles of a therapeutic nurse-client relationship and responds appropriately (e.g. openness, non-judgmental attitude, active listening, self-awareness, boundaries)
  • Promotes the client’s positive self‑concept (e.g. supporting cultural and spiritual preferences, validating the client’s strengths, promoting the use of effective coping techniques, building community capacity)
  • Establishes and maintains culturally sensitive therapeutic nurse/client relationships based on trust, respect, professional intimacy, empathy and power (CNO standard for nurse/patient therapeutic relationships) in accordance with the SJHH code of conduct.
  • Utilizes a systematic process in the assessment, planning, implementation and evaluation of care in accordance with SJHH policies, procedures and protocols, privacy legislation, professional standards, and all legal requirements.
  • Performs and documents holistic assessments (physical and mental health) with clients utilizing standardized screening/assessment tools as applicable to acquire information about client functioning and care needs from multiple sources, including client perspective, family and significant others to facilitate the plan and delivery of care
  • Utilizes standardized screening and assessment tools for identification, monitoring and evaluation of strengths, goals, needs and risk management (e.g. suicide risk, falls, risk of aggression, prevention of pressure ulcers, and addiction/concurrent disorders)
  • Continuously analyses clinical data from multiple sources in order to make critical decisions, plan interventions and takes action to ensure appropriate members of the inter‑professional team are informed.
  • Plans and delivers individualized care with each client/family and ensures plan of care is consistent with client’s concerns, needs and priorities.
  • Prepares and administers medications and provides treatments as prescribed or as required, in a safe and effective manner according to professional standards and medication policies. Evaluates and documents outcomes on interventions.
  • Applies evidence-based treatment modalities such as Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Interpersonal Therapy, Smoking Cessation and Motivational Interviewing across the continuum of care
  • Continuously analyses rapidly changing clinical presentations from multiple sources, to make decisions regarding levels of engagement and observation and medical interventions, as required.
  • Provides ongoing patient and family education to assist in building knowledge and capacity to manage illness in hospital and while living in the community
  • Performs team-leader or charge nurse role and responsibilities as assigned.
  • Engages in a collaborative partnership with Registered Practical Nurses, acting as a resource as required. The RN will actively intervene when level of client’s functioning changes in relation to risk, predictability and complexity beyond the Registered Practical Nurse’s scope of practice.
  • The RN will facilitate and co‑facilitate patient and family therapeutic groups as part of daily activities
