As an Occupational Therapist, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners. • Completes assessments of clients in a variety of treatment settings (private homes, schools, seniors residences, long-term care facilities, group homes) with analysis based on objective, subjective and functional findings.
• Establishes realistic, measurable, time-limited treatment goals and develops treatment plans.
• Ensures that occupational therapy treatments are initiated with informed consent in accordance with the Health Care Consent Act.
• Provides direct treatment and/or support and teaching to informal caregivers, school personnel and other health care professionals involved.
• Participates in case conferences as required and consults with other health care team members to foster a holistic approach to care.

• Completes reassessments to review patient progress and treatment goals and reports to the Care Coordinator and other health care team members to ensure continuity of care and identify additional resources or referrals as needed.
• Plans, in collaboration with the patient, family and Care Coordinator, for termination of service when goals are achieved or service is no longer required.
• Completes assessments and recommendations for adaptation aids, assistive devices and equipment, respecting Ontario Assistive Devices Program requirements where appropriate.
• Maintains patient records and documentation in accordance with the College of Occupational Therapists of Ontario and Ontario Health atHome Champlain policies and procedures.
• Acts as a resource by providing educational programs to staff, service providers, school personnel, long-term care facilities and community partners as requested.
• Attends and participates in occupational therapy team meetings and general staff meetings.
• Maintains up-to-date professional knowledge and skills; prioritizes services according to policy.

Hours of work: 8:30am to 4:30pm, Monday to Friday

FTE: 1.0

Length of Temporary Assignment: October 25th, 2025

Salary Range: $44.480/hr to $51.340/hr

• Degree or diploma in Occupational Therapy and current certificate of registration with the College of Occupational Therapists of Ontario
• Related professional experience normally attained over two years in an Occupational Therapy role
• Related experience in a similar community-based setting is an asset
• Current CPR-C certificate

• Knowledge of the health care system and the role of Ontario Health atHome Champlain within the system
• Knowledge of Ontario Health atHome Champlain operations, quality, risk, and performance management principles
• Knowledge of relevant health care legislation
• Knowledge of Ontario Health atHome Champlain policies, procedures, strategic direction and goals
• Understanding of Ontario Health atHome Champlain partners and contracted service providers in the community

• Strong interpersonal skills to work with diverse patient groups
• Ability to manage difficult conversations in a healthy and effective manner
• Ability to work effectively in a team environment and independently
• Effective communication to establish and maintain contacts within the community
• Strong customer service skills and active listening and facilitation abilities
• Ability to deliver information in one-on-one, team settings and during presentations
• Effective planning and organizing to manage multiple patients and reports
• Accurate documentation and reporting skills
• Working knowledge of computer software (email, internet) and Microsoft Office
• Valid driver’s license and access to a vehicle
• Fluency in English and French is an asset
• Vulnerable Sector Check completed within the last six months

Hours of Work: 35 hours/week (Mon-Fri, 8:30am-4:30pm)
