Qualifications

• Minimum of 1 year of experience as an RPN, preferably in long-term care or a healthcare setting.
• Strong knowledge of geriatric care and chronic disease management.
• Ability to assess conditions and recognize changes to provide appropriate interventions.

Responsibilities

• Provide direct nursing care and monitor health status of residents.
• Collaborate with healthcare team to create and evaluate care plans.
• Educate residents and families on health conditions and wellness strategies.
