The Thames Valley Family Health Team (TVFHT) is one of the largest family health teams in Ontario, committed to delivering high-quality, patient-centered care across our communities. As part of our Interprofessional Primary Care Team (IPCT) expansion initiative, we are excited to introduce a new Physician Assistant (PA) role to help increase access to care, strengthen attachment for patients without a primary care provider, and enhance population health outcomes. We are seeking a collaborative, passionate, and motivated Physician Assistant (PA) to join our primary health care team. This exciting new position offers a unique opportunity to demonstrate the value and scope of the PA role within an interdisciplinary primary care model. The PA will play a key role in increasing access to care, supporting patient attachment, and enhancing the quality and efficiency of care for patients without a regular primary care provider.

Position Summary

The PA will work under the direct supervision of our physician partners in the IPCT expansion initiative, and work collaboratively with NPs, and interdisciplinary team of health professionals to provide comprehensive primary care. This role offers a unique opportunity to demonstrate the value and scope of the PA profession within primary care, contributing to prevention, chronic disease management, health promotion, and episodic care for individuals and families across the Thames Valley region.

Key Responsibilities

  • Provide comprehensive patient assessments, including medical history , physical examinations, and diagnostic planning.
  • Assist in the diagnosis and management of a wide range of acute and chronic illnesses and injuries.
  • Perform controlled acts such as interpreting diagnostic tests, including laboratory and imaging studies under the supervision of physicians.
  • Develop and implement evidence-based treatment plans in collaboration with the interdisciplinary team.
  • Administer medications and perform procedures as directed by supervising physicians
  • Provide education and support to patients and their families regarding disease prevention, chronic disease management, health promotion and treatment options
  • Participate in interdisciplinary care planning, case conferences
  • Maintain accurate and up-to-date electronic medical records for all patient interactions
  • Support care transitions and follow-up with patients recently discharged from hospitals or emergency departments.
  • Contribute to organizational initiatives aimed at improving attachment, access to care and population health outcomes.
  • Support quality improvement activities and participate in professional development and training.

Qualifications

Graduate of an accredited Physician Assistant education program

Current certification as a Physician Assistant in Canada (CCPA) with active CPSO licensure

  • Experience in primary care, family medicine, or community health settings is considered an asset.
  • Strong knowledge of medical terminology and procedures related to various specialties
  • Strong clinical skills, sound judgment, and the ability to work both independently and collaboratively.
  • Excellent communication and interpersonal skills.
  • Proficient in the use of electronic medical records (EMR) and other digital tools.
  • Commitment to patient-centered care and ongoing professional development.

Reporting & Supervision

The Physician Assistant will work under the supervision of a physician, reporting directly to the Director of Clinical Services, and in close collaboration with other team members. Administrative support and performance coaching will be provided by the leadership team.

Location & Hours of Work

This is a full-time ongoing on-site position. The successful candidate will be based within one of our primary care sites or co-located with one of our community partners in the Thames Valley region, with occasional travel to other locations within the region.

Hourly Rate: $80,535 - $98,214 (dependent on experience)
