Salary: $63.66-$74.46/hour

Position Summary:

The Ambulatory Program Nurse Practitioner is accountable to the Manager of Ambulatory Program. The Ambulatory Program Nurse Practitioner is accountable to the Manager of Ambulatory Program and supports several Ambulatory Programs including but not limited to: the Nurse Practitioner clinic for unattached patients, Internal Medicine clinics, Medical Day Unit, and the Systemic Treatment Program.

The Ambulatory Nurse Practitioner possesses a master’s degree in nursing from an approved Nurse Practitioner (NP) program in accordance with the College of Nurses of Ontario. The NP demonstrates a comprehensive knowledge in Ambulatory Care sensitive conditions.

The NP works in collaboration with the interdisciplinary team and health care providers within the hospital and larger community. The NP manages, directs, and provides comprehensive care for a select patient population within the ambulatory program, ensuring clear communication is provided to primary health care providers and referring health care providers to ensure seamless transitions in care. In partnership with the interdisciplinary team and organizational leadership, the NP supports and leads quality improvements within the Ambulatory Program.

EDUCATION AND EXPERIENCE:

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class with one of the following specialty classes: Adult or Primary Health Care
  • Master of Science in Nursing or Masters in a relevant discipline
  • Experience working within an outpatient setting
  • Experience working within an acute care environment
  • Experience with chronic disease management
  • Membership with a professional organization such as NPAO, RNAO, or CNA
  • Minimum of 3 years nursing experience in respective area required
  • Demonstrated ability to deal with and resolve conflict
  • Strong critical thinking and problem solving skills
  • Demonstrated ability to work independently and as a collaborative member of a team
  • Must be able to meet the physical demands of the position
  • Double vaccinated with a Canadian recognized COVID vaccine
  • Excellent attendance and availability required
