JOB TITLE: Licensed Practical Nurse (LPN) 7002
REPORTS TO: Nursing Manager
JOB SUMMARY: In keeping with the Mission, Vision, and Values of Park Place, the LPN collaborates with the interdisciplinary team to use the nursing process and apply nursing knowledge, skills and clinical judgment to perform assessments, plan, implement, and provide quality, person-centred nursing care to Residents. The LPN operates in accordance with the competency guidelines and full scope of practice as outlined by the provincial governing body’s practice standards and Code of Ethics and Park Place policies and procedures, in a setting where client care needs are established and changes are predictable, with a commitment to Park Place’s culture of safety.
Duties and Responsibilities
- 1. Nursing Knowledge
- 1.1 Demonstrates knowledge and ability to utilize the nursing process to identify Residents’ health care needs and work toward mutually agreed health outcomes.
- 1.2 Demonstrates critical thinking and problem-solving skills in all aspects of care.
- 1.3 Demonstrates ability to prioritize and manage Resident care according to identified care needs.
- 1.4 Provides support to Residents and their families and establishes a therapeutic relationship with them.
- 1.5 Demonstrates computer skills to document Resident care, share information with the health care team while respecting confidentiality, and validate evidence-based practice.

- 2. Nursing Practice
- 2.1 Assessment and Care Planning
- 2.1.1 Utilize standardized assessment tools based on identified care needs and best practices.
- 2.1.2 Manage move‑in/move‑out processes and Resident transfers, including preparing comprehensive health and psychosocial assessments, processing orders, documenting and reporting, providing health teachings, medication reconciliation, care planning, and advance care planning.
- 2.1.3 Develop individualized care plans in collaboration with the Resident, family, and interdisciplinary team.
- 2.1.4 Document health assessments and pertinent clinical information accurately in the Resident’s electronic health record/chart.
- 2.2 Medication Management
- 2.2.1 Understand pharmacology principles and medication administration.
- 2.2.2 Perform assessments prior to medication administration and follow the rights and checks to ensure safety.
- 2.2.3 Administer medications per policies and procedures, monitor effectiveness, and document outcomes.
- 2.2.4 Initiate, discontinue and manage hypodermoclysis infusions as ordered.
- 2.2.5 Maintain records of accurate drug counts for controlled medications.
- 2.2.6 Report adverse reactions and medication errors per provincial legislation and Park Place policies.
- 2.3 Performs, monitors and evaluates nursing procedures and interventions (e.g., head-to-toe assessments, vital signs, ADLs, nutrition/hydration, wound care, palliative and post-mortem care) as outlined in provincial LPN competencies.
- 2.4 Communication, Documentation and Technology
- 3.1 Communicate fluently in English (verbal and written).
- 3.2 Apply therapeutic communication techniques.
- 3.3 Document and update care information to ensure Resident safety and comfort.
- 3.4 Share information with the health care team while respecting confidentiality.
- 3.5 Perform related clerical duties (e.g., phone, appointments, ordering supplies, diagnostic requisitions).
- 3.6 Nursing informatics: understand impact of informatics/tech in health care, be willing to learn and adapt to electronic processes, monitor care documentation in the EHR, complete UDAs, and report incidents and orders accurately in the electronic system.
- 3.7 Demonstrate knowledge of risks associated with inappropriate use of social media.
- 3. Health and Safety Responsibilities
- 4.1 Read, understand and comply with OHS policies and safe work practices.
- 4.2 Actively participate in the Health and Safety Program (safe procedures, policy promotion, safety suggestions, minutes).
- 4.3 Take reasonable precautions to protect safety of Residents, self, co‑workers and the public by following safety policies and emergency procedures.
- 4.4 Report near misses, injuries and equipment damage and complete required reports.
- 4.5 Correct or report unsafe conditions and malfunctions to the Nursing Manager or designate.
- 3. Professionalism and Leadership
- 5.1 Maintain professional presence and behavior.
- 5.2 Develop professional relationships with the interdisciplinary team based on mutual respect.
- 5.3 Establish and maintain professional boundaries with Residents and their families.
- 5.4 Demonstrate conflict resolution to foster collaboration and trust.
- 5.5 Supervise staff by scheduling, evaluating performance and determining training needs, including care assignment for unregulated providers.
- 5.6 Organize workload and manage time effectively to complete assigned tasks.
- 5.7 Facilitate orientation and knowledge sharing with new staff, students, and team members.
- 5.8 Participate in continuous quality improvement, program evaluation, education sessions, team meetings and committees.
- 5.9 Work effectively in teams and supervise others respectfully.
- 5.10 Assess learning needs and pursue professional growth to maintain high nursing standards.
- 5.11 Perform other related duties as assigned.
Qualifications
- Education: Graduation from a recognized LPN program.
- Experience: At least one year of recent related experience.
- Certification/Registration: Active registration with the applicable provincial college (e.g., BCCNP, CLPNA) and current First Aid certification as required by provincial legislation.
Equipment Used
Computer, iPad, iPod, telephone, facsimile and photocopier; Vital signs machine; Mechanical lift; Medication and wound cart.
Physical Demands
The LPN position requires lifting, walking, reaching, bending, pushing, standing and sitting. Typical day includes 2 to 6 hours of standing or walking. Lifting up to 50 lbs, with frequent lifting of 20 lbs. Ability to manage stressful situations with dexterity and professionalism.
Health and Safety Risks – Environment
Physical strain (shoulder, neck, back), minor cuts/scrapes; exposure to dust, draughts; single-occupancy scenarios; chemical cleaners; exposure to communicable diseases; minimal radiation. PPE and safety measures apply.
Hours of Work
Hours of work: 1500-2300 (7.5 hours per shift). Biweekly rotation as scheduled (refers to shift blocks in schedule); no table-based schedule shown here.
