Job Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will have a strong background in patient care and be skilled in providing nursing support. As an LPN, you will play a vital role in delivering high-quality care to patients of all ages, ensuring their comfort and well-being.

Responsibilities
  • To provide nursing care to patients in collaboration with physicians, allied health and administration team.
  • Attend and participate in care team meetings monthly, to exchange information and discuss challenging cases for patients in your care.
  • Receive direction from family physicians and office manager.
  • Participate in the planning, education, and evaluation of prenatal patients, well babies, and patients with chronic diseases.
  • Call and book patients for chronic disease management visits as per the appropriate guidelines.
  • Call patients regarding results as tasked by physicians
  • Perform wound care procedures, ear syringing, immunizations, BP checks, foot checks, and educate patients in accordance with the LPN scope of practice.
  • Documentation in MedAccess EMR of all aspects of care
  • Production of reports as required by the practice group
  • Share in observing changes in patients’ condition and collaborate with Physicians as required.
  • Responsible for co-ordination and ordering of immunizations, collaborate with public health, temperature log of vaccine fridge
  • Keep medical supplies on hand in co-operation with the office manager
  • Responsible for stocking clinical spaces
  • Liaise with pharmaceutical representatives for lunch and learns.
  • Responsible for sterilization of equipment
  • Responsible for managing sample cupboard
  • Scheduling, prioritizing and time management skills are essential
  • These job duties are only the basic and prime duties of the job. Related and allied duties can and will be assigned from time to time as they may become necessary to the operation of the collaborative practice.
Qualifications
  • Currently licensed as an LPN in Nova Scotia.
  • Current certificate in Standard First Aid, including CPR.
  • Non-violent Crisis Intervention training an asset.
  • Vaccination Course is an asset.
  • Related experience preferred.
