The Care Company is a home health care organization committed to helping individuals live safely and independently at home. We work closely with hospitals, community partners, and interdisciplinary teams to support timely transitions from hospital to home and reduce avoidable readmissions.

We are expanding a specialized community-based program supporting adults with complex medical and functional needs following hospital discharge.

About The Role

We are seeking an experienced Occupational Therapist (OT) to provide in-home and virtual occupational therapy services across the Central East region.

This role is well suited for clinicians who are comfortable working in fast-paced, medically complex environments, value autonomy, and are experienced in hospital-to-home transitions.

Referral volume may fluctuate, and services are delivered within assigned geographic zones. Scheduling and coordination are managed centrally to support clinical efficiency and sustainability.

Key Responsibilities
  • Complete initial in-home occupational therapy assessments following hospital discharge or referral
  • Assess functional mobility, ADLs/IADLs, cognition, home safety, and environmental risks
  • Develop and implement individualized care and treatment plans
  • Provide education and coaching to patients and caregivers on functional strategies, fall prevention, and safety
  • Recommend, trial, and support access to assistive devices and home modifications, including ADP where applicable
Follow-up Responsibilities
  • Deliver focused follow-up visits to support goal progression
  • Document assessments, progress notes, and care plans in a clear, timely, and clinically compliant manner
  • Collaborate with an interdisciplinary care team including nurses, PSWs, and care coordinators
  • Communicate changes in patient status to the care coordination team promptly
Scheduling & Practice Model
  • Community-based practice within assigned geographic zones
  • Flexible availability based on patient flow and clinical need
  • Timely service initiation following referral
  • Visits scheduled to minimize unnecessary travel
  • No fixed clinic hours
Required Qualifications
  • Registered Occupational Therapist in good standing with the College of Occupational Therapists of Ontario (COTO)
  • Experience in home and community care or hospital discharge planning
  • Strong functional assessment and safety evaluation skills
  • Ability to work independently in the community
  • Reliable transportation within assigned region
  • Strong electronic documentation skills
Preferred Qualifications
  • Experience supporting medically complex or chronically ill adults
  • ADP Authorizer status
  • Experience collaborating with hospital teams or community care coordinators
Benefits
Compensation
  • Up to $750 per day
    • This is calculated according to a schedule of a minimum of 5 new clients per day, at $150 per initial assessment
  • $75 per in-person follow-up visit
  • $50 per virtual follow-up visit
