Job description

Are you an RN or RPN who leads by example in meeting or exceeding clients’ expectations? Do you want to use your supervisory background to promote excellence in clinical service delivery? If so, you’re the type of professional who will excel as part of the CBI Home Health team in Peterborough / Durham.

As a Care Supervisor for CBI's specialized, integrated care program within Peterborough & Durham regions, you will lead a group of direct care providers to ensure that clinical service delivery meets or exceeds standards set by CBI Home Health and external stakeholders. Showing the way as a Regulated Health Professional (RN or RPN), you will supervise regulated staff (RN/RPN) in the provision of services that align with their scope of practice, with a focus on ensuring service and clinical excellence. The Nursing Care Supervisor is a critical role that manages the day-to-day operations of several programs and is a primary point of contact between clients, field staff and managers.

Key Responsibilities
  • Complete a comprehensive initial and interim care assessment within expected timelines as per the contractual agreement (and as applicable to service program) utilizing tools, forms, and processes as aligned with CBI Home Health policy and procedure, guidelines, documenting all required information as well as any information pertinent to the client status and care planning.
  • Ensure client and care provider safety by using risk assessment tools provided
  • Respond to client complaints striving to resolve when able and escalating as required internally (to management) and externally (to funders) as appropriate.
  • Identify opportunities to improve service and/or service delivery, engaging appropriate team members to drive effective and efficient change as required.
  • Ensure the client/client family experience is satisfactory and ensure the client/client family is provided with all required information relevant to their care.
  • Collaborate with the interdisciplinary team and internal/external stakeholders on service delivery delegations in order to ensure care needs of the client and quality standards are being met.
  • Lead by example by promoting service delivery excellence through effective onboarding, mentoring/coaching, and managing performance of field staff (including conducting and documenting 30-60-90 day check-ins and annual performance reviews).
  • Lead regular team meetings and/or individual meetings, and send regular communication to field staff regarding pertinent updates as required.
  • Ensure all care provider team members are fulfilling accountabilities as per their job description and are meeting expectations, through regular touch points and observing performance at drop-in visits as required.
  • Ensure all reporting (internal and external) meet or exceed all standards and expectations of CBI Home Health, stakeholders, and the associated regulatory body (if applicable).
  • Ensure appropriate documentation is completed accurately and on time including client charting as per associated regulatory body and funder agreements.
  • Complete review and follow up of practice management system reports to monitor and assess client documentation, GPS tracking and visit compliance.
What You Will Need To Be Successful
  • Possess a current registration with College of Nurses of Ontario (CNO) as a Registered Nurse or Registered Practical Nurse
  • Have 2 years of supervisory experience and/or experience in the home health service delivery
  • Computer skills – Proficiency with Microsoft Office Suite, ability to effectively use MS Teams and scheduling systems
  • Be able to effectively establish and build rapport with clients and care staff
  • Possess a valid Ontario driver’s license with access to a reliable vehicle with ability to travel in community
  • Have the ability to provide best possible solutions to problems that align with policies and uphold best practices
  • Be self‑motivated with an ability to work both independently and as an effective member of a team.
