RPN / RN Nurse Care Manager (Care Designer) – Full-time
Location: Mississauga, Ontario
Address: 89 Queensway W, Mississauga, ON (next door to Mississauga Hospital)
Schedule: Flexible availability, including evenings, weekends, holidays, and on-call duties. Schedule provided 2 months in advance.
Responsibilities
- Consult Conversion
- Conduct Caring Consults to assess physical, emotional, and environmental needs.
- Advise on client care and achieve 90% consult-to-client conversion.
- Build long-term relationships with community leaders and professional organizations for referrals.
- Client Care Delivery & Experience
- Attend all First Visits with caregivers to ensure quality care and alignment with service agreements.
- Conduct supervisory visits to monitor caregiver fit and care quality.
- Develop and update individualized care plans.
- Educate clients on self-care and monitor risks / outcomes.
- People Leadership
- Lead caregiver team: hiring, training, goal setting, and ongoing coaching.
- Ensure caregiver availability aligns with business needs.
- Oversee continuing education, including Discovery Day and monthly in-services.
- Conduct performance and job satisfaction reviews to maintain an employee Net Promoter Score (NPS) of 9+.
- Scheduling Collaboration
- Work with scheduling team to adjust caregiver assignments and ensure client coverage.
- Step in to assist with scheduling when required.
- Monitor attendance via AlayaCare (Attendance Monitoring System) with a target below 5%.
- Administration
- Maintain accurate client care plans, caregiver files, payroll, and billing records.
- Conduct quarterly audits to ensure 100% accuracy.
- Ensure compliance with health, safety, and employment legislation.
- Approve visits and premiums for payroll / billing.
- Key Metrics
- Consult Conversion Rate: 90%
- Driving Revenue Growth
- NPS (Client & Employee Scores): 9+
- Attendance Monitoring (AMS): Below 5%
Qualifications
- Registered Practical Nurse (RPN) or Registered Nurse (RN), licensed in Ontario.
- CPR and First Aid certification.
- Valid driver's license and access to a car.
- Strong computer skills (G-Suite required; AlayaCare an asset).
- Excellent written and verbal communication skills.
- Highly organized with strong time management and multitasking skills.
- Background check with Vulnerable Sector Search (renewed every 5 years).
- Negative TB test (renewed every 5 years).
