A governmental body in Canada is seeking a part-time permanent Admissions and Assessment Coordinator based in Whitehorse, Yukon. This role involves managing client assessments and admissions in continuing care services. This opportunity will be used to fill ONE (1) Part‑Time Permanent position starting March 25, 2026, working 60 hours bi‑weekly. It will also be used for anticipatory term or full‑time permanent positions and will establish a 6‑month eligibility list. The job is based in the thriving city of Whitehorse in the beautiful Yukon Territory.

The Mission of the Yukon Home Care Program is to provide client‑focused health services in the home, improving quality of life and supporting Yukoners to live independently. The Government of Yukon’s Home Care program enables Yukoners of all ages to receive high‑quality, evidence‑based health and support services while remaining in their homes and communities. The program collaborates closely with medical facilities, community partners, First Nations governments, and other government programs to ensure care is coordinated, supporting successful transitions in care.
The Admissions and Assessment Coordinator team manages and facilitates client assessments and admissions across various continuing care services. These services include permanent and respite admissions in a long‑term care home, as well as services offered through the Community Day Program.
This position reports to the Home Care Senior Manager, Strategic Operations. The role is central to the client intake process for Continuing Care services, and involves reviewing and prioritizing referrals, completing comprehensive client assessments, and identifying complex care needs and risks to assist with determining appropriate levels of care for clients.
Responsibilities also include facilitating client admissions and collaborating with a variety of internal and external stakeholders to support successful transitions in care. The position may also provide interim care coordination for clients not admitted to the home care program who are awaiting admission to programs such as respite care or permanent long‑term care.
