Job description
  1. Responsible for providing and coordinating patient care, educating patients and the public about various health conditions, and providing advice and emotional support to patients and their family members.
  2. Assess and identify patients' needs and implement and monitor the patient's medical plan and treatment. They work with a team of physicians and other health care specialists or allied health in various settings.
  3. Assessment and Evaluation
    1. Meet the essential mental and physical health needs of people of all ages and conditions, including those under end-of-life care.
    2. Perform systematic patient assessment and collect pertinent data about the patient’s health or situation.
    3. Diagnosis
      1. Understand and analyze the assessment information and data
      2. Determine actual and potential nursing diagnoses, problems, and issues.
    4. Outcomes Identification
      1. Define the predicted and expected outcomes of the patient’s planned care.
      2. Create and evaluate patient care plans with the medical team.
    5. Planning
      1. Define the care plan in collaboration with the healthcare team, patients, and families.
      2. Maintain and update the technical and clinical skills related to nursing practice.
      3. Employ strategies to promote patients’ health and a safe environment.
      4. Understand different diagnostic tests and laboratory exams.
    6. Intervention & Implementation
      1. Develop a plan of care for each patient.
      2. Assess and manage all patient populations with appropriate intervention.
      3. Insert, manage, and discontinue intravenous catheters and intraurethral catheters.
      4. Record vital signs, recognize, and address abnormalities.
      5. Perform essential life support (BLS)
      6. Support patient with personal care activities such as feeding, dressing, and performing hygiene.
      7. Prepare patients for medical examination and treatment.
      8. Administer medications and monitor patients for any side effects and reactions.
      9. Administer treatments such as IV fluids or blood products and monitor patients for any reactions.
      10. Provide wound care management.
      11. Assist in medical procedures invasive or non-invasive as needed
      12. Collect blood samples and other body fluids specimens for laboratory work and assisting for stem cell collection.
      13. Operate and monitor medical equipment.
      14. Facilitate the process of safe patient’s admission and discharge.
      15. Transfer patient safely into and out of bed and chair
      16. Provide patient care while maintaining the basic principle of medicine of “do no harm.
      17. Record details and symptoms of the patient medical history and current health
      18. Promote patient optimum health and wellbeing.
      19. Identify patients' care requirements and implement the identified and expected patient’s plan.
      20. Participate in decision-making concerning patients’ care as well as coordinate their care with other healthcare professionals and facilities.
      21. Maintain a safe environment using quality assurance and risk management strategies.
      22. Provide a safe environment for patients and staff, including implementing infection control procedures.
      23. Participate in continuous healthcare related quality improvement activities.
      24. Reflect on practice outcomes and make changes to practice when appropriate.
      25. Evaluates patient’s treatment outcome.
      26. Engage in proper documentation relevant to patient’s care.
    7. Plans, implements and evaluates nursing care given to assigned patients in an effective and organized manner.
    8. Performs initial assessment of the patient
    9. Monitors vital signs of mother and fetus throughout the delivery phase and initiate fetal heart monitor, performs vaginal exams, interpret fetal heart tones and determine frequency duration and intensity of contractions
    10. Monitor progress of laboring mothers, interpret fetal heart monitor and notify physician when appropriate for progress or problems.
    11. Assist mother with breathing exercises, pain management techniques and administer pain medication as ordered.
    12. Predominant role in preparation, administration and documentation of medications
    13. Prepare birthing room for delivery using strict sterile technique.
    14. Prepare mothers for elective or emergency Cesarean Section by following written C-Section preoperative orders of the physician. Provide emotional support to mother, obtain proper consents and pre-op diagnostic studies and coordinate smooth transfer to surgery if indicated
    15. Assist physician in artificial rupture of membranes, initiation of internal fetal monitoring or intrauterine pressure catheter, initiation of Pitocin drip or other IV medications, sterile vaginal exams or other procedures.
    16. Assist anesthetist with the administration of spinal or epidural anesthetic and thereafter maintaining proper documentation of the same.
    17. Perform post-partum care and provide health education to mother: check lochia, episiotomy and fundus, provide instruction in child care, breast feedings and self-care to mother, monitor vital signs per policy, and report problems to physician.
    18. Maintains and updates all relevant patient documentations/medical records as per hospital policy and procedures.
    19. Maintains confidentiality of patient’s records
    20. Responsible for patient care programs like MAMA CARE, exclusive breast feeding etc.
    21. Collect and send documents required for the birth notification to the medical record section
    22. Maintenance of labor and delivery register
    23. Participates in the rehabilitation and discharge planning of patients/families.
    24. Assists with transfer of patients within the health care facilities and serves as transport nurse for patients during ambulance transport
    25. Encourages an economical approach to the utilization of equipment’s and supplies while assisting the Charge nurse in maintaining an appropriate ward stock
    26. Follows the reporting mechanisms/ chain of command in the Nursing Department. Ensuring patient care issues are correctly and quickly reported.
    27. Takes responsibility for professional clinical practice and is actively involved in updating and continuing education.
    28. Competent with cardio-pulmonary resuscitation technique
    29. Responsible for checking of crash carts and emergency equipment’s as per hospital policy
    30. Responsible for maintaining inventory checklist of medications including high alert medications and narcotics as per hospital policy
    31. Responsible for maintaining inventory checklist of Hazmat items including MSDS as per hospital policy
    32. Ensures preventative and ongoing maintenance of equipment is carried out, reporting all failures or deficiencies through appropriate department head
    33. Participates in the implementation of unit goals, objectives, protocols, standards, policies and procedures that are consistent with those of the nursing service and the hospital
    34. Takes responsibility for the ward / unit in the absence of the Charge nurse as assigned by the nursing administration
    35. Demonstrates sound knowledge and participates in fire safety, patient safety, infection control and the other necessary precautions as per hospital policy
    36. Demonstrates strict adherence to all hospital policies and procedures
    37. Carries out other duties as instructed by the Nursing Shift Supervisor / Nursing Director.
    38. Responsible for implementing health and safety measures related to the Pandemic for themselves and others by their actions or omissions, and continue the provision of health-care during a pandemic
    39. Responsible to comply with the managements instructions and policies, i.e. follow safe work practices, use equipment, wear protective equipment, participate in training, and report hazards or injury, etc.
    40. Responsible to monitor their own health and immediately report any issues of concern to their HOD.
