A healthcare service provider in the City of London is seeking a dedicated Practice Nurse to deliver comprehensive nursing services. Responsibilities include implementing health promotion programs, managing chronic diseases, and collaborating with multidisciplinary teams. The ideal candidate will be a registered nurse with experience in primary care, specifically in chronic disease management. This role offers the opportunity to work in a supportive environment focused on patient care and health improvement.
To be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well woman and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
