Qualifications

Main duties of the job3>
  • Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
  • Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
  • Signposting to appropriate care providers (including primary and tertiary care).
  • Raising concerns to senior staff for support and management.
  • Completing documentation and planning care for patients.
  • Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
  • Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.
