A community mental health organization in Eastbourne is seeking a motivated individual to join their team. The role involves supporting service users with mental health needs, facilitating activities, and being part of a multidisciplinary team to promote recovery and independence. Candidates should have experience in mental health services and NVQ level 3 or equivalent. An exciting opportunity has arisen to join the bank for the East Sussex Rehabilitation Pathway.
The East Sussex Rehabilitation Pathway provides specialist community and inpatient mental health rehab services for adults over the age of 18, living within East Sussex. We believe in providing outstanding quality, safe and effective care and treatment that everyone can be proud of.
The ward works closely with Community Rehabilitation colleagues as an integral part of theaward-winning East Sussex Rehabilitation Pathway.
The purpose of this post is to work as a member of a team which provides mental health services, and to focus on the day to day support needs of service users. The post holder will provide support to an allocated group of service users with more complex needs, promote recovery and maintain their independence within the community.
The post holder will work with other members of the multidisciplinary team to facilitate the development and delivery of community care. This includes contributing to the full range of care tasks, assisting the care co-ordinator to assess and plan, implement and evaluate individual care plans, co-ordinating work under supervision, taking on the role of care co-ordinator under supervision, co-facilitating therapeutic groups and individual activities, information gathering and analysis to inform a multi-disciplinary formulation, audit and specific aspects of therapeutic intervention.
