Salary: £26,976 - £30,653 (Pro rata salary £23,910 - £27,169). Incremental progression throughout the band is available.

We are delighted to be recruiting a Transitions Facilitator to join our Learning Support Team. Transitioning to college is a significant milestone in any young person’s life. As a Transitions Facilitator, you will play a vital role in supporting students with Special Educational Needs and Disabilities (SEND) as they prepare to join further education.

You will attend Education, Health and Care Plan (EHCP) Annual Reviews across the region, meeting with students, parents/carers, and schools. You’ll serve as a key point of contact, offering guidance and support throughout the transition process.

We are looking for an innovative and proactive individual with outstanding communication skills and the ability to work independently. A strong understanding of SEND and the potential challenges young people face when starting at a new educational setting is essential.

Above all, you will be empathetic and committed to advocating for young people, helping them to overcome barriers and achieve their educational aspirations.
