Job description
Overview

We are flexible on individual post holders up to 37 hours. This may include hours from 08.00 – 20.00 and will include hours over 7 days. We are recruiting Band 4 TAPs to support Frailty Services in NHS Forth Valley. These post holders will work alongside physiotherapists, occupational therapists, speech and language therapists and dietitians to provide care to the frail population. The role involves contributing to pathways between community, front door and downstream wards as part of a period of change in how we manage frail patients across the NHS Forth Valley system. You will be expected to be mobile across the geographic area of Forth Valley and to explore your role in frailty across these areas.

Responsibilities
  • Responsible for the initial assessment and treatment of the frail population, which may include prevention of admission and discharge planning.
  • Collaborate with multidisciplinary colleagues to deliver care and improve services.
  • Support staff wellbeing and contribute to service improvements within the frailty pathway.
Qualifications and Requirements
  • Commitment to working across a seven-day week with potential extended hours (8am–8pm).
  • Willingness to contribute to changing frailty pathways and to work across community, front door and ward settings.
  • Experience and skills relevant to frailty care and collaborative practice with therapists from multiple disciplines.
  • The successful candidate must be a member of the Protecting Vulnerable Groups (PVG) Scheme.
