A regional housing support organization is seeking a Support Planner to assist individuals facing homelessness in North Lincolnshire. In this permanent role, you'll provide empathetic support, assess needs, and enable Service Users to engage in meaningful activities. SUPPORT PLANNER - RSAP NORTH LINCOLNSHIRE, SCUNTHORPE / LINCOLN, PERMANENT, 37HRS, GRADE 3, £25,235 , FTE

We provide accommodation for those rough sleeping / sofa surfing either in a hostel or dispersed environment. Our aim is to meet their support needs and help the individual overcome challenges which could result in further homelessness.

Duties and Responsibilities
  • To provide support to Service Users in a safe and maintained environment, by support planning, reviewing and assessing their individual risks and needs.
  • Be responsible for assessing and evaluating the needs of Service Users.
  • To ensure that tasks identified in support plans are communicated to Support Workers and that these tasks are clear, appropriate and agreed with the Service User.
  • To enable Service Users to pursue meaningful activities and improve their overall well‑being.
