Specialist Nurse (Care Transfer Hub)

The Northern Hospital Discharge Team and Single Point of Access Team have been combined to form the Care Transfer Hub North. This presents an exciting opportunity for a Band 6 to join the new team. You will have experience of working with patients in both acute and community settings and discharge planning from an acute hospital setting. Broad clinical experience and confidence, innovation and collaboration are valuable assets for this role.

Job responsibilities

The role will be varied and is expected to develop as the team adapts to a new model of working and aims to improve the discharge process for patients requiring care after leaving hospital. Responsibilities include assessing, planning and reviewing care, understanding the range of care available to avoid hospital admission and to support discharge, and identifying and resolving delays in discharge by understanding hospital discharge pathways and access points.

We are a friendly and supportive team that works closely with colleagues from Health and Social Care in both acute and community settings. Working pattern
