You will collaborate with an Occupational Therapist and commissioning colleagues to provide stellar support to service users accessing Supported Living services. This position is full-time and offers a rewarding hourly rate of £38. Over the course of 3 months, you’ll contribute to maximizing the independence of our service users, ensuring their support is perfectly tailored to their needs.

Responsibilities

• Conduct assessments and reviews for adults in supported living, ensuring care packages are tailored to meet individual needs.
• Collaborate with providers to ensure commissioned hours are appropriate and resources offer value for money.
• Optimize funding resources through effective case management and the development of personalized support plans that promote independence and wellbeing.
• Work closely with multi-disciplinary teams, service providers, and service users to achieve positive outcomes.
• Engage in regular on-site presence at the Civic Offices, Reading for essential meetings and site visits, while balancing remote work throughout the week.
