What you will do
  • Assess the needs of children with disabilities and their families, developing and implementing tailored care plans.
  • Collaborate with multidisciplinary teams to deliver cohesive and comprehensive support services.
  • Advocate for the rights and needs of children, ensuring they have access to appropriate resources and opportunities.
  • Conduct home visits and maintain accurate records to monitor progress and adapt care plans as required.
  • Provide guidance and advice to families and caregivers, fostering resilience and empowerment through challenging circumstances.

Able to start ASAP, this role based at Moor Lane, KT9, is a maternity cover up until mid-2026. The role requires attendance at the office in Moor Lane three days a week; however, for the induction period over two weeks, you will be required to be in the office five days a week. Preferably, candidates will have experience working with children with disabilities.
