Social Worker- Adults within Community Team in Tavistock, Devon earning £31–£35 hourly. This ongoing full-time locum position brings an exciting opportunity for an experienced social worker ready to embrace the charm and challenges of working in the vibrant community of Tavistock. You will be a crucial member of our Adult Community Team, working with adults aged 18 and over to support their journey towards independence and well-being.

Responsibilities
  • Undertake Care Act Assessments to assess and support the needs of adults in the community.
  • Carry out Mental Capacity assessments and Best Interest outcomes, ensuring decisions are in the best interests of individuals.
  • Complete Deprivation of Liberty Safeguards (DoLS) assessments with proficiency.
  • Navigate Continuing Healthcare (CHC) checklists and Decision Support Tools (DSTs) as part of assessment processes.
  • Work from a Strengths-Based perspective and demonstrate Least Restrictive Practice in all interventions.
  • Support the team duty function by triaging referrals and prioritising daily case management activities.
  • Collaborate confidently with professional partners in a multidisciplinary team (MDT) approach.
