Job Description

Job Title: QSW - Adults - Community Care (Learning Disabilities)

Pay 32LTD inclusive or 24 PAYE exclusive per hour

Job Purpose:

To deliver high-quality, person-centred social work practice in line with the Care Act 2014 and associated legislation. The post holder will manage a caseload of adults with learning disabilities and autism, undertake statutory duties, and contribute to safeguarding, risk management, and service improvement.

Key Responsibilities:

Care Management and Casework:

Undertake the full range of Care Act responsibilities, including assessments, support planning, reviews, and risk assessments.

Manage a caseload of complex cases, ensuring timely and accurate record-keeping using systems such as Mosaic.

Carry out statutory duties under relevant legislation, including the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS).

Participate in the team duty system, responding to new referrals, emergencies, and safeguarding concerns.

Undertake safeguarding enquiries in line with local multi-agency procedures and the principles of Making Safeguarding Personal.

Identify and assess risk, completing risk assessments in accordance with Merton Care Management Procedures.

Complete Care Act assessments, support planning, reviews, and risk assessments.

Respond to referrals and emergencies via the team duty system.

Essential Requirements:

Qualified Social Worker with current registration (e.g., Social Work England)

Experience in adult social care, particularly with learning disabilities and autism

Strong understanding of the Care Act, Mental Capacity Act, and safeguarding legislation

Ability to manage complex caseloads and work under pressure

Excellent communication, negotiation, and interpersonal skills

Commitment to person-centred, strengths-based, and evidence-informed practice

Contract Details:

Contract Length: 13.0 months
