Job Description

Job Title: QSW - Adults - Community Care (Learning Disabilities)

Pay 32LTD inclusive or 24 PAYE exclusive per hour

Job Purpose:

To deliver high-quality, person-centred social work practice in line with the Care Act 2014 and associated legislation. The post holder will manage a caseload of adults with learning disabilities and autism, undertake statutory duties, and contribute to safeguarding, risk management, and service improvement.

Key Responsibilities:

  • Care Management and Casework:
  • Undertake the full range of Care Act responsibilities, including assessments, support planning, reviews, and risk assessments.
  • Manage a caseload of complex cases, ensuring timely and accurate record-keeping using systems such as Mosaic.
  • Carry out statutory duties under relevant legislation, including the Mental Capacity Act and Deprivation of Liberty Safeguards (DoLS).
  • Participate in the team duty system, responding to new referrals, emergencies, and safeguarding concerns.
  • Undertake safeguarding enquiries in line with local multi-agency procedures and the principles of Making Safeguarding Personal.
  • Identify and assess risk, completing risk assessments in accordance with Merton Care Management Procedures.
  • Complete Care Act assessments, support planning, reviews, and risk assessments.
  • Respond to referrals and emergencies via the team duty system.
  • Essential Requirements:

  • Qualified Social Worker with current registration (e.g., Social Work England)
  • Experience in adult social care, particularly with learning disabilities and autism
  • Strong understanding of the Care Act, Mental Capacity Act, and safeguarding legislation
  • Ability to manage complex caseloads and work under pressure
  • Excellent communication, negotiation, and interpersonal skills
  • Commitment to person-centred, strengths-based, and evidence-informed practice
  • Contract Details:

  • Contract Length: 13.0 months
