Job description

The post holder is a Senior Physiotherapist (band 6) with the clinical skills, expertise and knowledge to support community reablement, 2 day urgent, community Physio & Falls service within Wokingham Intermediate Care. Intermediate care is a multidisciplinary service that helps people to be as independent as possible, providing support and rehabilitation to at risk patients or those who have been in hospital. It aims to ensure people transfer from hospital to the community in a timely way and to prevent unnecessary admissions to hospitals and residential care.

The Wokingham Intermediate Care team consists of Physiotherapists, Occupational Therapists, and Senior Therapy Assistants, who treat people in their own homes to rehabilitate them to as high a level of independence as possible. The multidisciplinary team works with patients across Wokingham in close communication with social services, voluntary agencies, and GPs. This post involves 7‑day working within a shift pattern within the intermediate service and requires timely assessment and management of referrals with multiple co‑morbidities and complex rehabilitation needs.

Responsibilities
  • Support triage the referrals for intermediate care services
  • Provision of equipment to promote independence and safety
  • Hold a caseload and work autonomously
  • Work flexibly to ensure urgent referrals are adequately managed
  • Support colleagues in the delivery of compassionate, safe, and structured care
  • Provide guidance and appropriate delegation within the team
  • Participate in service development and quality improvement
  • Support the supervision of more junior members of staff and provision of education and clinical physiotherapy advice to staff/student in Intermediate Care
