Job description

The post holder will act as an experienced physiotherapist, helping to develop integrated health care teams in the community with in a Multi-Disciplinary Team (MDT). The post holder will provide high quality physiotherapy interventions to a range of patients from different community settings such as: frailty, rapid response / admission avoidance, intermediate care, falls prevention through to preventative action and support for self-management of long term conditions. The caseload will include advanced assessment and treatment of patients, provision of individual and group education and treatment, provision of assistive devices (e.g. orthotics) and equipment as well as liaising with others services and agencies.

Main duties of the job

To manage a defined caseload using evidence based /client centred principles to assess, plan, implement and evaluate interventions, maintaining associated records.

To provide a high standard physiotherapy service to the specialist caseload across various sites within Tower Hamlet as the service requires.

To perform advanced physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions to provide a diagnosis and develop and deliver an individualized treatment program.

To provide leadership for more junior staff, as agreed with senior staff.

To deputies for the Senior Physiotherapists in their absence as required.

To supervise Physiotherapy students on clinical placements.

To participate in CPD and develop skills and knowledge through participation in the in service training scheme.

To undertake lone working when required.

To be able to work flexibly and participate in potential future 7 day working developments (when required)

Clinical Responsibilities

To be professionally and legally be accountable for all aspects of own work, including the management of patients in your care

To undertake all aspects of clinical duties as an autonomous practitioner

To be responsible for maintaining accurate and comprehensive patient treatment records in line with legal requirements and Health & Care Professions Council (HCPC) standards of practice

To understand the legal framework around gaining informed consent for treatment and to implement appropriate procedures in line with trust policies

To have a comprehensive understanding of patients clinical presentations based upon advanced clinical knowledge

To develop specialist knowledge of particular conditions and patient presentations specific to different diagnoses and treatments

To undertake a comprehensive subjective and objective assessment of patients with diverse or complex presentation or multiple pathologies and use advanced clinical reasoning skills to accurately identify impairments, abilities and participation

To develop timely and comprehensive discharge plans, considering appropriate future intervention in accordance with prognosis

To assess clinical risk within own patient caseload and to be aware of risks involved in the caseloads of other team members

To formulate, plan, deliver, evaluate and progress a patient-centred physiotherapy treatment and management program by effective and efficient clinical reasoning, using current evidence based practice (e.g. Best Practice Statements, journal review, in-service training, post-graduate training, peer review)

To use valid and reliable outcome measures to evaluate patient progress, reassessing and adjusting treatment programs as indicated

To assess and treat appropriate patients on a weekend day. This will include patients needing not only treatment for respiratory issues but also patients who require rehabilitation and long term management.

To work closely with the multi-disciplinary team, acknowledging areas of uni- and inter-Professional expertise, ensuring optimal out-come for both patients and service

To be an active multi-disciplinary team member, ensuring a coordinated patient-centred approach to service delivery. This will include multi-disciplinary team discussion of patient care, prognosis, goal attainment and timely discharge planning

To develop timely and comprehensive discharge plans, considering appropriate future intervention in accordance with prognosis

To liaise with, and provide advice and education to, medical, nursing staff, social services staff and other AHPs regarding the 24-hour management of patients on a daily basis and in all emergency duty situations

To identify change in patient presentation and arrange for urgent or non-urgent review by other disciplines

To be responsible for safe and competent use of all equipment, appliances and aids by both patients and staff under your supervision

To consistently maintain best practice, Continuing Professional Development and Career Pathway through, for example, attendance at postgraduate courses, in- service training, journal club and Special Interest Groups, encouraging staff under your supervision do likewise.

To take responsibility for participating in PDP and meeting set objectives.

To be participate actively in in-service training programs, tutorials, Clinical Governance days, peer review and courses, both internal and external

To participate in, and/or lead departmental and multi-disciplinary audit and research initiatives into specific areas of clinical practice utilizing support and supervision from senior staff as required.

To undertake evidence-based audit and/or research projects to further own and teams clinical practice within each rotation

Organisational

To be responsible for organizing and planning own case-load to meet service and patient priorities, allowing adequate time for non-clinical duties, such as report writing, leasing within the team, across services and other agencies, and student supervision

To deputies for the Band 7 in their absence on an agreed, short-term basis, taking responsibility for operational management of the team (juniors, assistants, students and any A&C staff), allocating and organizing the work-load to meet service priorities

To prioritize the team work-load as temporary staffing shortages arise, e.g. short term sick leave

To be flexible during the working day, allowing response to unexpected situations, e.g. acute patient illness, urgent referrals

To assist in the induction of more junior staff members and students

The CHTs if required by the population of Tower Hamlets, work over extended days (8am 10pm) including weekends and the post holder would need to be able to work occasional shifts within these times as required by the CHTs.
