Job summary

We have an exciting opportunity for a Band 6 Senior Community Nurse who has a passion for admission prevention to join a core Team of ACPs to deliver a new and developing countywide Hospital at Home (H@H) service in Worcestershire.

This is a full time post, but we would consider applicants looking for part time. The post is based in the Single Point of Access (SPA) Worcester and will be the clinical coordinator for the H@H service. You will be managing referrals in and out of the service as well as managing the daily capacity and caseloads. There will be the opportunity to do clinical visits if required to support delivery of the service and patient care as required.

This is an exciting opportunity to be part of the delivery of a new service and to play a significant part in shaping and developing this service. There will be the opportunity to partake in audit and service development as well as the clinical duties of the post. There will also be the opportunity to work collaboratively with stakeholders, in particular with Acute Trust colleagues. The service will be countywide across Worcestershire, so the ability to travel across the county is expected. The coordinator role will cover 09:00–17:00 seven days a week including bank holidays, so shift patterns are to be arranged to enable cover of operational hours.

Main duties

Support the assessment and management of acutely unwell patients in the community who might otherwise require admission to hospital, with the support of a dedicated medical model for Hospital @ Home.

Work independently to offer urgent care, treat minor illness, provide patient advice and support chronic disease management. Prioritise and triage patient needs, make referrals for investigations as appropriate in line with local and practice guidelines, and escalate to clinical leadership as required.

Job responsibilities

You will treat patients with acute exacerbations of chronic diseases, manage long term conditions, and support the assessment, planning, and treatment of patients to prevent hospital admission. You should have excellent clinical, written, and verbal communication skills, be able to work independently, and manage your own workload. For purposes of the role, you must be NMC registered, hold a full UK driving licence, have good IT skills, and have excellent communication skills.
