We are seeking a compassionate, skilled, and motivated Band 6 Mental Health Nurse to join our Older Adult Community Mental Health Team within the Shropshire, Telford and Wrekin locality.

This is an exciting opportunity to be part of a dynamic and supportive multidisciplinary team delivering high‑quality care to older adults experiencing functional and/or organic mental health needs. You will work collaboratively with service users, carers, and partner agencies to promote recovery, independence, and quality of life.

As a Mental Health Practitioner within the Older Adult Community Mental Health Team, you will be responsible for delivering comprehensive assessments, risk formulations, and evidence‑based interventions to support older adults with complex mental health needs. The role involves working closely with colleagues across the multidisciplinary team and liaising with GPs, social care, and voluntary sector partners to ensure continuity and quality of care. You will maintain accurate and timely clinical records in line with Trust policies, contribute to service development and quality improvement initiatives, and provide professional advice, support, and supervision to colleagues where appropriate.

Main duties of the job
  • Take responsibility for the management of a complex caseload, including specialist assessment, care planning with service users and carers, and evaluation of individual treatment plans.
  • Participate in setting quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication in accordance with Trust procedures and NMC guidance.
  • Assess patients' individual care needs, develop, implement and evaluate care programmes.
  • Deliver a range of interventions in accordance with the agreed plan of care and pathway guidance, including pharmacological and psychologically informed interventions.
  • Contribute to risk assessments and risk management plans, promoting positive risk‑taking where appropriate to improve quality of life and maximise occupational performance and independence.
  • Maintain an effective reporting system, observing and reporting verbally and in writing on patient conditions.
  • Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process.

Qualifications
  • Registered mental health nurse on NMC Register
  • Post‑registration experience; evidence of formal courses and training at diploma/degree level
  • Registered mentor
  • BSc degree in nursing or equivalent level
  • Qualification/training in providing supervision or coaching
  • Non‑medical prescriber
  • Psychological intervention qualifications including EMDR, CBT, CBTp, family interventions, CAT, DBT
Experience
  • Evidence of extensive post‑registration experience working with adults experiencing mental health related difficulties
  • Experience working with people who have difficulty engaging with services and have complex mental health needs
  • Experience assessing, planning, coordinating and managing people with mental health problems in a community setting
  • Experience working with older adults
