A healthcare service provider is seeking a Care Co-ordinator to work in community services in Ellesmere Port. Job description

This is an exciting opportunity for a Care Co-ordinator to work across the neighbourhood community services covering the West Cheshire footprint. You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service. You will work alongside team and partners including GPs and the voluntary sector, to support patients within the care community.

All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. This post is a Permanent, full time 37.5 hours per week position, Monday to Friday 9-5pm and requires travel between CWP bases.
