A healthcare service provider is seeking a Care Co-ordinator to work in community services in Ellesmere Port. The role includes co-ordinating multi-disciplinary meetings and ensuring high-quality customer service. Ideal for those looking to support patients within a collaborative care community. The position is permanent, full-time (37.5 hours per week) with required travel between bases.
This is an exciting opportunity for a Care Co-ordinator to work across the neighbourhood community services covering the West Cheshire footprint. You will be a central co-ordination point for local multi-disciplinary activity, ensuring high quality customer service. You will work alongside team and partners including GPs and the voluntary sector, to support patients within the care community.

All new starters will commence in post on one fixed start date each month, in line with our Prepare to Care (P2C) induction programme date. P2C offers up to 3 weeks training to support you to meet the induction / initial training requirements of the role. This post is a Permanent, full time 37.5 hours per week position, Monday to Friday 9-5pm and requires travel between CWP bases.
