Job summary

Qualifications

  • Band 3 Healthcare Assistant level with a keen interest in rehabilitation/frailty and end-of-life care.
  • Must be able to read and reflect criteria from job description and person specification.

Responsibilities

  • Provide a high-quality person-centred approach to care delivery.
  • Deliver planned care to a group of patients in accordance with Trust policies.
  • Work effectively as part of a team and guide less experienced staff.
  • Manage a defined group of patients under the indirect supervision of a Registered Nurse.
  • Contribute to the ongoing assessment and planning of patient care.
  • Promote health and prevent ill health in service users.
  • Plan and undertake clinical tasks guided by procedures.
  • Undertake delegated clinical skills within scope of practice.
