Position

Rehabilitation Assistant Practitioner

An exciting opportunity has arisen for a Rehabilitation Assistant Practitioner within the North Community Rehabilitation Team based at the Douglas Grant Rehabilitation Centre at Ayrshire Central Hospital.

Hours and Shift Pattern

22.3 hours – days are negotiable. This post is offered on a permanent basis.

What you will do

Job Purpose

  • We are looking for a suitably experienced health care support worker to join our team to provide non-complex physical rehabilitation and falls screening within North Ayrshire.
  • The post holder will work within a multidisciplinary team who deliver adult community rehabilitation in North Ayrshire; the caseload is predominately older adults who are experiencing reduced function, falls and frailty.
  • You will work autonomously as part of a team delivering Rehabilitation and falls screening to individuals in their own homes.
  • To carry your own caseload and monitor patient progress within scope of practice; work is supervised by qualified Nursing or AHP staff.
  • To screen/assess patients and initiate and implement treatment programmes according to protocols and pathways set up by qualified Nursing or AHP staff.
  • Work without direct supervision, reporting back patient progress and informing qualified Nursing or AHP staff of any possible deviations from protocols/pathway.
  • To support qualified nursing, occupational therapy and physiotherapy staff with patients during visits and undertake appropriate delegated tasks.
  • To support the operation of the team by undertaking administration and data entry tasks.
