Overview

A Vacancy at East Suffolk and North Essex NHS Foundation Trust.

We are seeking to recruit a well organised, dedicated individual to join us as a Primary and Secondary Care Interface Facilitator based in Colchester.

Responsibilities
  • Lead and manage liaison activities between primary and secondary care providers, ensuring seamless, integrated service delivery.
  • Work to improve communication, streamline referral pathways, promote and facilitate effective use of both primary and secondary care resources, and promote patient-centred care across the healthcare system.
  • Excellеnt communication skills are essential as you will play a key role in facilitating and maintaining effective relationships between primary and secondary care providers.
  • This role will be primarily based at our Colchester Hospital site, and will include travelling to other ESNEFT sites and GP practices when required.
  • Act as the key liaison between primary care providers (GP practices, community teams) and secondary care (hospitals, specialist services).
  • Build and maintain effective partnerships with clinicians, service managers, and external stakeholders.
  • Lead service improvement initiatives, audits, and evaluations to ensure best practices are shared across care sectors.
  • Analyse referral data, patient outcomes, and other metrics to evaluate service efficiency.
  • Attend local interface meetings, integrated care board (ICB) sessions, and multidisciplinary team (MDT) discussions.
  • Develop and implement strategies to support integrated care pathways.
