South Cotswold Primary Care Network (PCN) is asuccessful network of 5 practices with a combined list size of over 64,000patients.

We are looking for an enthusiastic, organised andforward-thinking individual who wants to make a difference to the local GPpractices in the South Cotswolds and to the health of the local population.

Main duties of the job

The PCN manager will work alongside the GP ClinicalDirector for the South Cotswolds PCN, getting involved in all aspects ofmanagement and finance, supporting the PCN to deliver change, implement newservices, build relationships with local stakeholders, and meet all therequirements of the PCN Directed Enhanced Services (DES).

It is an exciting time to take on this role. TheNHS Ten Year Plan has been published and there is significant focus oncollaborative working with partners across the system as well as thedevelopment of Integrated Neighbourhood Teams (INTs).

The role has a real opportunity to shape the futureof how services are delivered across the five practices within the SouthCotswolds PCN, ensuring that it meets the requirements of the current DES,whilst identifying and driving opportunities for greater collaboration, to meetthe needs of the local patient populations.

We are looking for highly motivated candidates whohave demonstrable experience of successful management and project delivery,have strong financial acumen and are good at building relationships, workingwith a wide range of stakeholders, and have the drive and enthusiasm to makechange happen.

Although not essential, this post would certainlysuit a candidate with a healthcare management background. However, relevanttransferable skills, enthusiasm and a willingness to learn about our sector arefar more important.

PCNs are groups of GP practices working closelytogether - along with other healthcare staff and organisations in order toprovide joined up services to the local population. Within the South Cotswolds,we established a PCN in 2019, bringing together 5 GP Practices and covering apopulation of approximately 60,000. South Cotswold Primary Care Network is acollaborative project involving the following GP Practices:

Job Purpose

The PCN Manager role exists to support the deliveryof the Primary Care Network agenda and play a pivotal role in the delivery of high-qualityprimary health care within the South Cotswold Network. The PCN Manager isintegral working alongside the Clinical Director and the PCN Board members.

Coordinating the successful achievement of allgovernance requirements, the PCN Manager will work with the PCN to translatenational and local policy into meaningful changes, and will support practicesto achieve improved outcomes for the local population.

Travel between Network Practices and to othervenues for meetings and work will be required from time to time.

This job description is an outline of key tasks,responsibilities and outcomes required of the role. However, the remit of thisrole is broad and evolving and the job holder will carry out any other dutiesas may reasonably be required by the PCN Clinical Director. The following areasof responsibility are an indication of the role but could be subject to changeas the primary care network model evolves locally and nationally.

Primary Duties and Areas of Responsibility

Organisational:

Support the PCN Director to ensure the smoothrunning of the PCN.

Act as a first point of contact for incominginformation and queries related to the PCN.

Co-ordinate the delivery of enhanced services andother service submissions on behalf of the PCN.

Co-ordinate other PCN projects including the localCovid Vaccination Programme and Quality Improvement Projects.

Alongside the PCN Director and Board, manage anddevelop the Network including strategic and operational planning processes tosupport the PCNs objectives.

In liaison with the PCN Director, provide businessmanagement support (including financial and HR oversight) to Healthy Coriniumas required, a community interest company owned by the PCN.

Support the PCN Director with the writing ofbusiness and development plans as required.

Support recruitment, induction and training of newPCN staff where required.

Finance and Administration:

Responsible for the oversight of the Networksfinancial processes, payments to all the practices within the Network.

Calculation and reconciliation of payments to thePCN and to practices.

Work alongside Practice Managers within the Networkto set up financial systems and processes as required by the PCN.

Produce the Board Information Pack to ensure thatthe PCN Board has relevant information to make informed decisions.

Collate information from the 5 practices todemonstrate compliance with contractual requirements.

Monitor and check the accuracy of any submissionsfor Enhanced Services, contractual requirements on behalf of the Network andensure that all claims are submitted on a monthly and quarterly basis asrequired.

Manage contractual requirements on behalf of theNetwork.

Work in conjunction with the LMC and GPCollaborative, to ensure representation of the GP practices, as the National Neighbourhood HealthImplementation Programme (NNHIP) develops.

Ensure that the PCNs physical and financialresources are managed and maintained for maximum effectiveness through:

Ensuring financial risks arerecognised and mitigated through appropriate action

Financial planning and monitoringof expenditure

Supporting the practices,through managing all aspects of the ARRS and identifying significant deviationsfrom plan

Planning and monitoring incomeand cash flow to ensure that income is maximised and that funds are availableas required

Ensuring all claims aresubmitted in good time and payment made and received

Ensuring the practicesallocation of resources if fair and equitable

Present financial plans,monitoring information and reports to the PCN as required

Maintaining a copy of payments,receipts and invoices and working with the PCN Accountants to prepare AnnualAccounts.

Additional Roles Reimbursement Scheme (ARRS)-responsible for the coordination of the ARRS monthly submissions in a timelymanner for ICB review and approval- reconciliation of the payments- ongoingARRS budget planning support for practices.

COVID vaccination programme coordination across the5 practices- ordering vaccines, submitting payment claims, reconciliation ofpayments.

Further develop the access to Social Prescribingprovision- continue developing relationships with local organisations:voluntary, Council, to provide a more cohesive approach for patients.

Working closely with the Frailty service (HealthyCorinium), to provide the ICB Frailty Implementation Plan as part of theIntegrated Neighbourhood Teams (INT).

Quality and Governance:

Ensure the PCNs compliance with the GDPR and FOIalong with other relevant legislation and professional information managementstandards such as GMC/BMA and LMC guidance.

Be responsible for the production of performanceand quality reports within the Network as needed.

Collate feedback / analysis data on behalf of theNetwork to report to the Integrated Care System (ICS).

Ensure that the PCN Board and Clinical Directoradhere to the governance structure as set out in the Network Agreement.

Alert PCN members to issues of quality and risk.

Organise PCN Board Meetings and any other PCNmeetings as required. This includes booking rooms, agreeing agenda, taking andcirculating minutes.

Information Management & Technology

Support with the ongoing Modern General Practice-driving initiatives to encourage greater use of technology, including NHS appto provide access to patients in the best way that suits them.

Ensure the PCNs compliance with the GDPR and FreedomOf Information (FOI) along with other relevant legislation and professionalinformation management standards such as GMC/BMA and LMC guidance.

Oversee the administration of informationmanagement and technology of the PCN, including its shared folder structure andstaff access to this.

Ensure the integrity of the system working with theICS to implement their guidance.

Welfare:

Co-ordinate PCN training and development of staffin liaison with the PCN Education Lead, Clinical Director and Practices e.g.Protected Learning Time Events.

Working Relationships:

Work as a flexible member of the PCN providingsupport to other team members when necessary.

Represent the PCN in cross organisation meetingswhen agreed e.g., ICS / locality meetings.

Encourage supporting staff to collaborate throughsharing information and intelligence across different activities.

Build good relationships with all stakeholdersinvolved in the PCN.
