Job Overview

This is a position in primary care within the community mental health team in Conwy.

We provide assessments from various agencies referred through our SPOAA daily meetings, and offer brief interventions within groups and occasionally on a one-to-one basis.

Knowledge of third sector agencies is useful as we work to become more familiar with them and improve our signposting and working relationships.

Main duties of the job

An exciting opportunity has arisen in Primary Mental Health Support services for an experienced, motivated, and forward-thinking practitioner to join a busy supportive team in Conwy.

The successful candidate will work closely with a multidisciplinary team, receive referrals from GPs, and provide short-term interventions for individuals with mild to moderate illness. Collaboration with secondary care, specialist services, third sector organizations, and statutory agencies is essential.

Responsibilities include managing a caseload, formulating risk management plans, and care planning based on thorough assessments. The candidate should be a good team player with excellent communication skills and effective time management. Welsh language skills are desirable; applications in Welsh or English are equally welcome.
