Job summary

Were lookingfor a dedicated and proactive Practice Nurse to join our friendly andforward-thinking team at Avon Valley Practice, working across our Upavon andDurrington surgeries.

This is arewarding role providing essential primary care nursing services, supportingpatients with long-term conditions, and playing a key part in preventativehealth and wellbeing. The postholder will deliver a wide range of treatmentroom and chronic disease management clinics, contribute to health promotionactivity, and ensure patients receive safe, effective, and compassionate careat the right time. They will also support continuity of care for our morecomplex or frequently attending patients, helping them navigate their healthneeds with confidence and clarity.

Main duties of the job

What youlldo

  • Work closely with our GPs, nurses, HCAs andwider multidisciplinary team to deliver high-quality, evidence-based nursingcare across our Upavon and Durrington surgeries.
  • Provide a full range of treatment room andlong-term condition management clinics, supporting patients to improve theirphysical health, confidence and wellbeing.
  • Take a joint lead role in Infection Prevention& Control (IPC) alongside the Managing Partner, including audits,education, and ensuring compliance with national and CQC standards.
  • Provide mentoring, clinical supervision andsupport to junior nursing staff, HCAs, students and trainees, helping buildskills, confidence and a positive learning culture.
  • Contribute to proactive care and healthpromotion, ensuring patients with more complex needs or frequent attendancepatterns receive safe, coordinated and timely support.
