A leading healthcare organization in Walsall seeks an Advanced Care Physiotherapist to enhance patient care by assessing and managing musculoskeletal conditions independently. The ideal candidate will hold a degree in Physiotherapy and have significant clinical experience. Primary care is changing; both the way we interact with patients and the local structures. It is an exciting and dynamic time to work within General Practice. In Walsall, networks of local GP practices have formed seven Primary Care Networks (PCNs) to help improve the care offered to the local population and develop new ways of working. One of the new opportunities is that of Advanced Care Physiotherapists who can work in the role of First Contact Practitioner: providing clinical expertise, making independent decisions about patients' care, and seeing patients, without prior contact with their GP, in order to establish a swift diagnosis and management plan. We are seeking to recruit such individuals to work across GP practices in Walsall to improve care by streamlining the assessment and treatment process.

The minimum criteria for this position includes, but is not limited to;

Specific duties:
Clinical:
Work independently, without day to day supervision, to assess,diagnose, and manage patients presenting with MSK conditions presenting forthe first time to GP practices;
Deliver clinics in Primary Care where patients with musculoskeletalconditions can be seen and treated;
Make full use of the scope of Advanced Practice,utilising/developing skills relating to independent prescribing, injectiontherapy and investigation;
Make professional judgements and decisions in unpredictablesituations, including when provided with incomplete or contradictoryinformation and to take responsibility for making these decisions;
Develop integrated and tailored care programmes in partnership withpatients, providing a range of first line treatment options includingself-management, referral to rehabilitation focussed services and socialprescribing;
Manage complex caseloads, including working with patients with longterm conditions, co-morbidities, psychosocial and mental health needs,referring onwards as required and including social prescribing whenappropriate;
Communicate effectively with patients, and their carers whereapplicable, complex and sensitive information regarding diagnoses, pathology,prognosis and treatment choices supporting personalised care;
Request and follow up investigations (such as x-rays and bloodtests) and referrals to facilitate the diagnosis and choice of treatmentregime including, considering the limitations of these investigations,interpreting and acting on results and feedback to aid patients diagnosesand management plans;
Ensures care is proactive, preventive in focus and population based,with an emphasis on early intervention;
Maintain accurate and up-to-date patient records;
Maintain professional knowledge by keeping up-to-date withdevelopments in the field of musculoskeletal conditions/medicine.
Organisational:
Work as part of a multi-disciplinary team in a patient facing role,using the expert knowledge of movement and function issues, to createstronger links with wider services through clinical leadership, teaching andevaluation;
To contribute to developing and reviewing pathways of care formusculoskeletal patients in conjunction with the Community PhysiotherapyService, GPs, PCNs, Secondary Care Consultants, other Health CareProfessionals in line with the organisational, regional and nationalobjectives;
Provide leadership and support on MSK clinical and servicedevelopment across the PCN, alongside learning opportunities for the wholemulti-disciplinary team within primary care;
Develop relationships and a collaborative working approach acrossthe PCN, supporting the integration of pathways in primary care;
Encourage collaborative working across the wider health economy andbe a key contributor to supporting the development of physiotherapy clinicalservices across the PCN.
Governance and Audit:
Actively participate in clinical governance within the PCNs and themember practices;
Observe and maintain strict confidentiality with regards to anypatient / family / staff / records and information in line with therequirements of the Data Protection Act and Practices policies andprocedures;
Adhere to and participate with practices in infection controlpolicies and procedures;
Maintain professional registration with the Health and CareProfessions Council (HCPC);
Undertake mandatory training and any other training relevant to therole as required by PCN Practices or OurNet Health Services;
Undertake and/or participate in regular audits to monitor standardsas required;
Promote research and critical evaluation of all clinical practicesand ensure that service provision is evidence based.
Training and Development:
Take part in the training of other staff within the PCN relevant tothe role, including lecturing, teaching, training and supporting othermembers of the multidisciplinary team e.g. Practice Managers, GPs,receptionists.
Participate in accredited / validated courses to demonstratecontinuing professional development.
Be responsible for maintaining own competency by attendingin-service training, peer review, individual training and courses.
Undertake any other training, which may be relevant to the job roleand gain necessary skills to further develop the service.
