The Chalfonts Primary Care Network is seeking an experienced Clinical Pharmacist to join their multidisciplinary team (MDT).
Based at The Misbourne Practice (which comprises of The Misbourne Surgery in Chalfont St Peter and The St Giles Surgery in Chalfont St Giles), this role will be employed by The Chalfonts Primary Care Network, which includes three GP practices and supports a local population of approximately 31,000.
The post holder will provide expert clinical medicines management and support the management of long‑term conditions, working closely with the GPs and the wider Practice and PCN MDT team.
The role is suitable only for a pharmacist who has already completed the CPPE Primary Care Pharmacy Education Pathway and is able to work autonomously in a primary care setting.
The post holder will work within their clinical competencies and the requirements of the DES to deliver high quality, patient centred clinical pharmacy services, predominantly for The Misbourne Practice, with some flexibility to support wider PCN services according to need.
This will include supporting long‑term condition management (for example cardiovascular risk and CKD), undertaking medicines optimisation and safety work, contributing to repeat prescribing and medication review processes, and providing clinical support to the wider MDT through handling clinical queries and supporting practice audits and searches.
The Chalfonts PCN is a collaborative network of local GP practices (The Allan Practice, The Hall Practice and The Misbourne Practice) serving the Chalfont communities. The Misbourne Practice comprises of The Misbourne Surgery (Chalfont St Peter) and The St Giles Surgery (Chalfont St Giles), offering a friendly and supportive working environment with a strong focus on high quality, patient centred care.
The successful candidate will be part of an established PCN team and will work closely with other ARRS roles, to improve outcomes for our population.
